Clinical Comparative Analysis Of The Efficacy And Safety Of Laparoscopic Lurgery Versus Laparotomy Operation For Endometrial Cancer

Objective: To assess the efficacy and safety of laparoscopic surgery versus Laparotomy operation for endometrial cancer.Methods: Retrospective analysis 118 patients with endometrial carcinoma(according to FIGO 2009,preliminary diagnosis of stage I)after operative treatment in the attached hospital of Dalian Medical University between January 2010 to January2015.there were 13 cases in laparoscopic surgery group and 105 cases in laparotomy operation group,Follow-up time to January 2016,collectied two groups of patients cases information.By using SPSS software for analysis their operation time,intraoperatve blood soss,numbers of lymph nodes excised,anus exhausting time,hospital stays,Indwelling catheter time,intraoperative complications,postoperative complications recurrence rates,survival ratesResults: The operation time of Laparoscopic surgery group was234.2±35.1min,The operation time of laparotomy group was 183.3±26.2min,The difference was statistically significant(P<0.05),Laparoscopic surgery group time was longer than laparotomy group;the blood loss of Laparoscopic surgery group was292.3±107.1ml,the blood loss of laparotomy group was 324.5±87.2ml,There was no statistically significant difference between the two(P>0.05);the number of lymph nodes excised of Laparoscopic surgery group was 17.1±2.3,the number of lymph nodes excisedof laparotomy group was 19.6±3.2,There was no statistically significant difference between the two(P>0.05);there was 0 intraoperative complication in Laparoscopic surgery group and 1 in laparotomy group,There was no statistically significant difference between the two(P>0.05);The evacuation time of Laparoscopic surgery group was 1.8±0.6d,The evacuation time of laparotomy group was 2.1+0.7d,The difference was statistically significant(P<0.05),the exhaust time of Laparoscopic postoperative was earlier than laparotomy group;the Indwelling catheter timeof Laparoscopic surgery group was 2.4±0.5 d,the Indwelling catheter time of laparotomy group was 2.6±0.4 d,There was no statistically significant difference between the two(P>0.05);there was 1 postoperative complication in Laparoscopic surgery group and 3 in laparotomy group,There was no statistically significant difference between the two(P>0.05);The hospital stays of Laparoscopic surgery group was 9.3±1.5 d,The hospital stays of laparotomy group was 113.1±1.7d,The difference was statistically significant(P<0.05),the hospital stays of Laparoscopic group was shorter than laparotomy group;There was no statistically significant difference between their recurrence rates and survival rates(P>0.05).Conclusion: Compared to laparotomy operation,The use of laparoscopic surgery in treatment of endometrial cancer,which had the same number of lymph nodes excised,postoperative complication,recurrence rate,andsurvival rate after operation,can significantly reduce the blood loss of the operation,accelerate the postoperative exhaust time and reduce the intraoperative complications,and we still need more clinical data to evaluate the problems of long operation time.
