The Clinical Curature Effect Analysis Of The Internal Fixation To The Posterior Ankle Fractures Of Type Ⅱand Type Ⅲ

Objective: To explore the clinical effects of specific surgical options for posteri or ankle fracture internal fixation. Methods: 72 posterior ankle fracture patients w ere recruited from the Department of Orthopedics at Shandong Traditional Chinese Medicine Hospital from September 2011 to March 2015. According to Wang Zhish eng classifica-tion, 39 patients belong to type Ⅱ(A group) and 33 patients belong to type Ⅲ(B group). Two options of internal fixation, hollow screw fixation and plate fixation, were investigated. Type Ⅱincluded 27 patients with hollow screw fixation(A1 group) and 12 patients with plate fixation(A2 group), and type Ⅲi ncluded 17 patients(B1 group)and 16 patients(B2 group), respectively. The clinical effects of ho-llow screw fixation and plate fixation were evaluated based on AOF AS score, Baird-Jackson score, postoperative drainage volume and the du-ration of walking without crutch. Statistical analyses were performed by SPSS 21.0.Results: Between A1 and A2 groups, no statistically si-gnificant differences were observed i n aspects of AOFAS score, Baird-Jackson score, postoperative drainage volume and the duration of wal-king without crutch(P > 0.05). Remarkably, AOFAS score,Bai rd-Jacksonscore and the duration of walking without crutch were significantly more in B1 group than B2 group(P < 0.05). However, no significant difference was observed in postoperative drainage volume between B1 and B2 groups(P > 0.05).Conclusions: For type Ⅱposterior ankle fr-acture, there is no significant differencebetween hollow screw fixa-tion and plate fixation.However, plate fixation is superi or to hollowscrew fixation for type Ⅲposterior ankle fracture. In conclusion, we recommend plate fixation for severe posterior ankle fracture.
