| Objective: The thesis aims to probe into the correlation between lung governing dispersing and descending theory and pattern-based treatment of cough.Methods: Methods of literature research and clinical epidemiology investigation are employed in our study.1. Literature research: By collecting and classifying the related literature of previous generations, we summarize the correlation between lung governing dispersing and descending, and the occurrence and treatment of cough. Based on modern literature, we study the prescriptions, patterns and therapeutic methods of refractory cases of cough.2. Clinical epidemiology investigation:(1) We collect 80 cases of patients with cough from Zhonghua Medical Institution, Skudai, Johor state, Malaysia, including their general information, clinical manifestation, constitutional types. Based on the data, we identify the patterns of each patient and probe into the occurrence regularity and relations between patterns and body constitutions by statistical method.(2) We collect 140 cases of patient with cough from 4 state-owned outpatient departments of Johor, Selangor, Pahang and Penang, Malaysia, including 93 cases from Zhonghua Medical Institution of Skudai, and 15 cases from Zhon Gua Chinese Medicine Department of Batu Pahat, Johor State, 19 cases in Medicine Buddha Selangor Total Welfare Centers of Selangor, 8 cases from Koh Medical 和 Acupuncture Centre of Pahang, 5 cases from Shin Kang Chinese Medicine Centre of Penang. Based on the data, we identify the patterns of each patient and probe into the occurrence regularity and relations between patterns and body constitutions by statistical method.Results: 1. Literature research: Literature research shows that lung failing to govern dispersing and descending is closely related to the occurrence of cough, that lung failing to disperse is related to endogenous cough. However, other causes including diet and phlegm may also contribute to endogenous cough. 2. Clinical epidemiology investigation:(1) 140 cases are chosen to study the relations between cough and body constitution, among which balanced yin-yang constitution accounts for 60%, qi deficiency constitution 20.16%, yang deficiency constitution 17.74%. Occurrence of cough varies greatly between genders and ages, women aged from 41 to 50 being more susceptible. It is also related to northeast wind.(2) Another 80 cases are chosen to study the relations between cough and body constitution, among which balanced yin-yang constitution accounts for 71.25%, qi deficiency constitution 23.94%, yang deficiency constitution 19.71%. Occurrence of cough varies greatly between genders, and females are more susceptible than males, women aged from 41 to 50 being more susceptible. The occurrence of cough is also related to rain, southwest wind, cold, preference for pungency, and is manifested as poor appetite and yellow and dense phlegm clinically. Male patients with cough mainly manifest as pattern of phlegm-heat obstructing lung, account for 30.44%, and the second comes patterns of wind-cold invading lung and wind-heat invading lung. Male patients aged from 31 to 40 are inclined to manifest as pattern of phlegm-heat obstructing lung. Female patients with cough mainly manifest as patterns of phlegm-heat obstructing lung and wind-cold invading lung, accounting for 31.58%. Female patients aged from 41 to 50 are subject to pattern of phlegm-heat obstructing lung while patients aged over 61 are subject to pattern of wind-cold invading lung. The result shows that both male and female patients with cough mainly manifest as pattern of phlegm-heat obstructing lung. Patterns of cough are also related to body constitution. Patients with balanced yin-yang constitution mainly manifest as pattern of phlegm-heat obstructing lung, accounting for 35%, and the second is pattern of wind-cold invading lung; patients with qi deficiency constitution mainly manifest as pattern of wind-cold invading lung, accounting for 28.57%, and the second is pattern of phlegm-heat obstructing lung; patients with yang deficiency constitution mainly manifest as pattern of phlegm-heat obstructing lung, accounting for 46.15%, and the second is pattern of wind-cold invading lung. Statistics show that patients of cough with balanced yin-yang constitution often manifest as phlegm-heat obstructing lung in terms of pattern, and thus, the disorders of lungs and spleen are the key factors to the occurrence of cough in the above mentioned areas. |
