The Clinical Observation Of Calcium Sulfate Bone Cement Combined With Yishen Quyu Soup In Treating Steinberg Stage Ⅰ、Ⅱ Femoral Head Necrosis

Objective: To observe the clinic curative effect of calcium sulfate bone cement combined with Lichen Quay Soup in treating Steinberg StageⅠ,Ⅱ femoral head necrosis,explore the mechanism,Provide clinical basis for optimization of treatment for the disease。Methods: According to the topics included in the standard,randomly select 32 cases who has been diagnosed with Steinberg installment Ⅰ,Ⅱfemoral head necrosis from joint section of henna province hospital of traditional between May 2013 to May 2014.All the patient are single hip disease,they were randomly divided into A(experimental group)and B(control group).After the Core Decompression the A group were injected with Calcium sulfate bone cement,then the next day begin to take Lichen Quay Soup as a treatment,A daily dose,a month for a period of treatment,taking three courses totally.After the Core Decompression the B group plant in an proper size Tantalum rod.Postoperative routine between the two groups have no difference,the clinical observation continuous follow-up of 12 months after surgery.Do the general data collection,The observational index include:preoperative Harris score,postoperative(3 months,6 months,12months)Harris score;preoperative NRS score,postoperative(3 months,6 months,12months)NRS score。X-ray check regularly(3 months,6 months,12months),To evaluate imaging in patients with index。Establish a database,Statistical processing all data by SPSS 18.0 software package for data processing。Measurement data results to mean (?) + S,Each parameter comparison between the two groups using the t test.Count data using x~2 test,P < 0.05 means have statistical significance.Results: By comparing the result and analysis we found that:(1)NRS pain score 12 months after surgery: group A(2.31±0.873),group B(3.06±1.181),group A averaging less than group B,group A was statistically significant difference(t = 2.535,P < 0.05).(2)Harris hip score 12 months after surgery: group A(86.13±4.425),group B(80.94 + 5.092),The average score A is higher than that of group B,the result was statistically significant difference(t = 3.009,P < 0.05).(3)3 months after surgery compared with the preoperative Harris score,NRS has no statistical significance,the Harris score of experimental group t = 1.159,P = 0.264;The control group,t = 1.567,P = 0.138;NRS score group t = 1.732,P = 0.104;Control group t = 1.695,P = 1.695(4)6 months after surgery compared with the 3months Harris score,NRS has statistical significance,the Harris score of experimental group t =-9.427,P < 0.001;The control group,t = 6.953,< 6.953;NRS score group t = 3.884,P = 0.001;the control group T = 5.172,P < 0.001(5)A year after surgery compared 6months Harris score,NRS score has obviously improved and the comparison was statistically significant,Harris score of experimental group t = 8.103,P < 0.001;The control group,t =-5.477,P< 0.001;NRS score group t = 7.730,P < 0.001;T =-5.137,P < 0.001.The relatively worse effect of two groups of postoperative are Steinberg Ⅱ period of C type,both two groups of postoperative scores are higher than preoperative Harris hip function,difference have statistical meaning(group A,t = 10.817,P < 0.05;group B: t =-9.084,P < 0.05);Both two groups of postoperative hip NRS pain score were higher than that of preoperative,difference was statistically significant(group A: t = 8.092,P < 0.05;group B: t =-2.535,P < 0.05).This group A during the follow-up period was 1 case failed hip,collapse of the femoral head finally do the THA,hip survial rate is 93.75%;3 cases of group B finally do the THA,fail rate is 18.8%,hip survial rate 82.2%.Conclusion:Compared with the single Core Decompression,the Calcium sulfate bone cement combined with YIshen Quyu Soup in treating has more advantage.There is a certain effect that the group A has a better result in prevention of collapse of the femoral head as well as relieve patients’ pain and improve the prognosis of femoral head necrosis curative effect,still there is still a low rate of failure which lead to Hip arthritis,this part of can’t prevent to a THA surgery.
