Objective:To identify the significant independent risk factors for left atrial thrombosis after mitral valve replacement.Methods:1.The perioperative data of a total of 2672 cases of mural valve replacement from January 2011 to December 2015 were analyzed retrospectively;2.The 15 cases with left atrial thrombus after mitral valve replacement during the follow-up were set as the thrombosis group,while 38 cases without left atrial thrombus after mitral valve replacement were selected as the control group;3. Combine the previous studies and the characteristic of the patients to select clinical variables which may related to the formation of left atrial thrombosis after mitral valve replacement;4.The clinical variables were divided into measurement data and Count data, and then make single factor statistical analysis respectively. Logistic analyses were performed based on the data of the statistically significant single factors to locate the significant perioperative independent risk factors of left atrial thrombus after replacement of the mitral valve;5.Analyzed and summarized the result of this study.Results: 1. Univariate analysis revealed that the predict factors including hospitalization duration 、 blood transfusion during operation 、 cardiopulmonary bypass(CPB) 、postoperative atrial fibrillation(AF)、postoperative warfarin start time、postoperative international normalized ratio( INR)、postoperative prothrombin time(PT)、preoperative and postoperative left atrial diameter(LAD) 、 preoperative and postoperative left ventricular end-diastolic diameter(LVEDD) 、 preoperative and postoperative left ventricular ejection fraction(LVEF) 、 preoperative pulmonary artery systolic pressure(PASP) 、 preoperative mitral regurgitation(MR) 、 preoperative left atrial thrombus 、 postoperative mitral valve effective area;3.Logistic regression analysisshowed that the independent risk factors included preoperative left atrial thrombus 、postoperative atrial fibrillation、postoperative left atrial diameter and preoperative mitral regurgitation,Among them, postoperative left atrial diameter showed to be the possible risk.Conclusions: The independent risk factors related to the formation of left atrial thrombosis after mitral valve replacement are preoperative left atrial thrombus 、postoperative atrial fibrillation、left atrial diameter and preoperative mitral regurgitation.
