Object:Lymphoma is a malignant tumor of lymphoid hematopoietic system, and in China, its clinical characteristics vary a lot from that in Europe or America. The prognosis of lymphoma is depended on early diagnosis and lymph node metastasis,as other types of tumors. This research is to describe the clinical features of primary extra nodal lymphoma(PENL) and to analyze the difference between PENL, secondary extra nodal lymphoma and non extra nodal involvement lymphoma in clinical features and prognosis.Methods: A total of 274 lymphoma patients were recruited at initial diagnosis or recurrence from January 2010 to December 2014. The retrospective analysis included the general information, extra nodal involvement or not, pathological type,changes in the levels of leukocyte, erythrocyte, hemoglobin, platelets, albumin,lactate dehydrogenase before and after chemotherapy, the international prognostic index(IPI), grouping and clinical symptoms.The SPSS version 18.0 was used for statistical analysis, and a two-sided P-value of <0.05 was deemed statistically significant.Results: Among the 274 patients, 140(51.09%) cases were PENL, 85(31.02%)cases were secondary extra nodal lymphoma and the rest 49(17.90%) were non nodal extra nodal involvement lymphoma.In the 140 PENL, the gastrointestinal tract was the most common sites, followed by the nose, bone,tonsil and the central nervous system. In the 85 cases of secondary extra nodal lymphoma, multiple metastases were frequently observed. Among the whole cohort, non Hodgkin’s lymphoma(NHL) accounted for the largest proportion(93.8%), and particularly,diffuse large B cell lymphoma was more commom. There were more diffuse large B cell types in the gastrointestinal tract, while there were more NK / T cell types in the nose. Compared with PENL and non extra nodal involvement lymphoma, the clinical stage of secondary extra nodal lymphoma was more advanced, the IPI score was higher, and the evaluation of therapeutic effect was poor.As for the laboratory index, there was adverse effect on hemoglobin, rather than leukocyte,platelets or albumin in secondary extra nodal lymphoma. Meanwhile, there was no effect on leukocyte, hemoglobin, platelets or albumin in PENL and non extra nodal involvement lymphoma. Only a small number of patients showed a decrease in the level of one or three series of blood cells. For all the three types of lymphoma, the high levels of LDH, ESR, β2-MG and FE were evaluated more than the normal value.Conclusion:PENL accounted for a higher proportion of lymphoma.The primary node lymphoma is of great possibility in the multiple external invasion, which results in the advanced clinical stage, higher IPI score and poor prognosis.Therefore, it is of particular value in early diagnosis and early treatment among patients with lymph node and organ invasion to improve prognosis.
