Objective Explore the postoperative pulmonary complications of elderly patients with gastric cancer associated risk factors,in order to take the initiative to prevent.Methods From January 2010 to February 2016,clinical data of 59 elderly(above 80 year old) patients with gastric cancer were analyzed retrospectively.According to the literature and clinical experience in our department,considered gender, age, BMI, smoking history, hypertension, diabetes, basic heart diseases,basic lung diseases,low serum albumin leels,anemia,operation method,intraoperative blood loss,the operation time,the number of lymph node cleaned,gastrointestinal decompression time for the risk factors.According to the presence of pulmonary complications in 4 weeks after surgery can be divided into two groups,the patients with postoperative pulmonary complications included in the observation group,the rest of the patients included in the control group.Using chi-square test for the comparison between the two groups,P<0.05 was significant difference.Independent risk factors of screening using Logistic regression analysis,P<0.05 was significant difference.Results In this study,there were 29 patients in postoperative pulmonary complcations,49.2% of all into patients.The single factor analysis results show that elderly gastric cancer patients with postoperative pulmonary complications was concerned with primary lung disease, BMI>25kg/m2, preoperative serum albumin <30g/L, intraoperative blood loss 400 ml factors,the difference was statistically significant(P<0.05).the result of multiple factors analysis method of Logistic show that primary lung disease, BMI>25kg/m2, preoperative serum albumin <30g/L,intraoperative blood loss 400 ml, were independent risk factors of postoperative pulmonary complications in elderly patients with gastric cancer(P<0.05,OR= 14.11,18.67,8.68,16.44 respectively).Conclusion For elderly patients with gastric cancer,the Major risk factors of PPC included Primary lung diseases, high BMI, low serum albumin leels in preoperative, intraoperative blood loss.For these related risk factors,targeted interventions can reduce the happening of the PPC.Age, gender, anemia, hypertension,basic heart diseases,operation method, indwelling time of nasogastric tube, tumor site, pathological type, infiltration depth, the number of lymph node cleaned in operation, lymph node metastasis,has no obvious correlation with PPC of elderly gastic cancer patients.Whether there is a connection between smoking,diabetes,long operation time and the PPC of elderly gastric cancer patients, still needs further research.
