Objectives Analyzing patients’ changes between preoperative and postoperative values, between pre-bone transportion and post-bone transportion values and its clinical implications of the lower limbs blood circulation were carried out through continuous monitoring the skin temperature of first toe web and the oxygen saturation of second toe of both sides for patients with tibial longitudinal distraction osteogenesis.Methods 25 cases patients hospitalized in The 1st Department of Hand Surgery from March 2014 to January 2016 were treated with tibial longitudinal distraction osteogenesis due to bone defect or nonunion of tibia caused by the trauma or other reasons. According to the patency of anterior tibial artery, posterior tibial artery(or fibular artery) postoperatively, all patients were graphed into normal group(17 patients), the anterior tibial artery injury group(5 patients), the posterior tibial artery or fibular artery injury group(3 patients). All patients approved to continuous monitor the skin temperature of first toe web. Calculate and record the difference between ipsilateral and contralateral(ipsilateral subtraction contralateral) and the oxygen saturation of second toe of both sides daily until the patient is discharged. All discharged patients were required to monitor its lower extremity skin temperature and oxygen saturation at every 2 weeks apart. All measured data were recorded and analyzed by SPSS19.0 statistical analysis software.Results(1) Skin temperature difference: All patients in the 3 groups were followed up, to some extent. the different values of skin temperature between the ipsilateral side and contralateral side were increased after the 2th day of surgery. the different values at preoperative point and the 2th day after surgery in the 2 group associated with vascular injury were less than the group without vascular injury. all patients in 3 groups, the most significant increasement of different values of skin temperature was early distraction osteogenesis state.(2) The blood oxygen saturation: The blood oxygen saturation was increased after the 7th day of surgery. All the 3 groups were identical in same period. To all patients in 3 groups, the most significant increasement of different values of blood oxygen saturation was early distraction osteogenesis state, the value of the affected limb was maintained at 97% to 99%, while the healthy side has been 97% to 99% numerical.Conclusions(1) The tibial longitudinal distraction osteogenesis can promote microcirculation reconstruction;(2) The velocity of angiogenesis was different in different periods;(3) The limb blood flow does not affect the reconstruction of microcirculation;(4) Limb local blood circulation was an important factor affecting the speed and quality of mineralization.
