The Expression And Clinical Significance Of Soluble B7-H3 In Human Urothelial Cell Carcinoma Of Bladder

Objective To explore the expression level and clinical significance of soluble B7-H3 in human urothelial cell carcinoma(UCC) serum and urine.Methods Using ELISA kits to detect soluble B7-H3 expression in UCC,papillary urothelial neoplasms of low malignant potential(PUNLMP),papillary urothelial neoplasms,and healthy volunteers,then evaluate the relationship between the s B7-H3 expression and clinical factor of UCC of bladder by combining clinical characteristics.Further we assess the value of soluble B7-H3 as a diagnostic marker in UCC of bladder.Results 1. There was a soluble form of B7-H3(s B7-H3) in the serum and urine of healthy volunteers and patients with urothelial neoplasms of bladder. The expression level of s B7-H3 in the serum in UCC, PUNLMP,papillary urothelial neoplasms and healthy volunteers were 22.868±3.439 ng/ml,20.329±2.768 ng/ml,19.818±2.836 ng/ml and 19.312±2.147 ng/ml.The expression level of s B7-H3 in the urine in UCC,PUNLMP,papillary urothelial neoplasms and healthy volunteers were9.059±2.346 ng/ml,6.972±2.275 ng/ml,6.443±1.650 ng/ml and 6.199±1.521 ng/ml.These results indicated that the expression level of s B7-H3 in UCC of bladder was higher than the PUNLMP,papillary urothelial neoplasms and healthy volunteers(P<0.01).However,The expression level of s B7-H3 has no significant difference on PUNLMP,papillary urothelial neoplasms and healthy volunteers(P>0.05).2.There were positive regulation between the high level expression of s B7-H3 and the histological grade,infiltrating degree,lymph node metastasis and distant metastasis of UCC of bladder.While the high level expression of s B7-H3 unrelated with gender and ages.3.To assess the value of s B7-H3 as a biomaker, the univariate ROC curves were used to calculate the sensitivity and specificity of this marker in separating patients with UCC from healthy subjects.An ares under the curve(AUC) value for s B7-H3 in the serum and urine respectively reached 0.796 and 0.834.Using a cutoff of 20.788ng/ml,the sensitivity and specificity of s B7-H3 in the serum in differentiating between patients with UCC and healthy volunteers was 72.10% and 68.90%.The sensitivity and specificity of s B7-H3 in the urine was 80.30% and 76.80% using a cutoff of 7.076ng/ml.Conclusion 1.We revealed the existence of a soluble form of B7-H3 in the serum and urine of healthy volunteers and patients with urothelial neoplasms of bladder. The level of s B7-H3 expression in UCC was higher than the PUNLMP,papillary urothelial neoplasms and healthy volunteers.There were positive regulation between the high level expression of s B7-H3 and the histological grade,infiltrating degree,lymph node metastasis and distant metastasis of UCC of bladder.s B7-H3 may paticipate in the invasion and metastasis of UCC and as a positive role in tumor immunity.2. s B7-H3 was a promising biomarker which may help to improve UCC diagnosis and prognostic assessment as it was significantly related with the progression of UCC of bladder.
