Part Ⅰ The Analysis of Dynamic Contrast-Enhanced Magnetic Resonance Imaging at 3.0T Study of Histology Characteristics and Clinical Stages of Cervical CancerObjective:To correlate pharmacokinetic parameters of 3.0 Tesla(T) dynamic contrast-enhanced(DCE-MRI) with the histology characteristics and clinical stages of cervical cancer. Methods:Tolly 40 healthy people and 88 patients with biopsy-proven cervical carcinoma who underwent DCE-MRI at 3.0T from Jan,2013 to Dec,2014. Then obtained the pharmacokinetic parameters(Ktrans 、 Ve 、 Kep),analysed the relationship of parameters between normal cervices and cervical cancer, squamous cell carcinoma and adenocarcinoma of the cervix, analysed the correlation between parameters and clinical stages and pathological grading of Cervical Cancer. Results:Ktrans、Ve and kep were significantly higher(p=0.000)in cervical cancer than in normal cervices.Ktrans was significantly higher(p=0.010)in adenocarcinoma cancer than in squamous carcinoma, the kep and Ve parameters were not significantly associated with different pathological types(p=0.076,p=0.112,respectively).Ktrans and kep were significantly higher(p=0.001,p=0.013,respectively)with the clinical stage progressed,Ve was not significantly associated with different clinical stage(p=0.071).there are a low positive correlation between Ktrans、Ve and clinical stage(r=0.395、0.307,respectively). Ktrans and kep were significantly lower(p=0.011,p=0.019,respectively) with the pathological grade progressed,Ve was not significantly associated with different pathological grade(p= 0.342).there are a weak negative correlation between Ktrans、Kep and pathological grade(r=-0.277、-0.289,respectively).Conclusion:DCE-MRI is feasible to assess microvascular permeability of cervical cancer patients. It can reflect the histology characteristics and clinical stage of cervical cancer in some degree as a useful addition in the diagnosis and selecting therapeutic schedule of cervical cancer. Part Ⅱ Correlation Between the Quantitative Parameter Values of dynamic contrast enhanced MRI and Clinic Stage in Cervical Squamous CarcinomaObjective To investigate the correlations between pharmacokinetic parameters of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI) and clinic stage in cervical squamous carcinoma. Methods DCE-MRI was retrospective analyzed in 81 patients with biopsy-proven cervical squamous carcinoma from Jan,2013 to Dec,2014. Then obtained the pharmacokinetic parameters Ktrans,Kep,ve,investigate the correlations between pharmacokinetic parameters of DCE-MRI and clinic stage. Results There was a middle positive correlation between Ktrans and clinic stage(r=0.445,p=0.000),there was a low positive correlation between Ktrans,ve and clinic stage(r=0.294,p=0.008;r=0.233, p=0.036;respectively).Conclusion There exist correlations between pharmacokinetic parameters of DCE-MRI and clinic stage in cervical squamous carcinoma,it can provide reference value for preoperative staging of cervical squamous carcinoma.
