Analysis On Clinical Application Of Human Albumin Injection In A Hospital In 2014-2015

ObjectiveA preliminary study of the clinical application of Human Albumin Injection, reviews of medical records and analysis of irrational drug use provide a reference for clinical medication and medical records reviews which achieves the aims of rational drug use, mitigating patients burden and optimum usage of albumin preparation resources. MethodsExtract 5617 electronic medical records in 2014 ~ 2015 years from a tertiary general hospital, including all the medical records related to Human Albumin Injection; then explore the overall usage situation of Human Albumin Injection from the term of patients age, annual usage amount, distribution of department using Human Albumin Injection, usage of each patient and distribution of disease, etc..; afterwards, review some medical records referring Human Albumin and evaluate the rationality of clinical application from the aspects of serum albumin levels, classification of diseases and reason of irrational use. ResultsIn the 5617 patient records, 292 are for less than 1years old infant patients, 82% is for 41-90 years old patients, 57% is for more than 60 years old patients.; for less than 1 years old infants, cardiac surgery and treatment of neonatal hyperbilirubinemia is mian evidence for the use of serum albumin; the use of Human Albumin relates to different departments and a wide range of diseases, and almost all departments in the hospital are using Human Albumin;Human Albumin dose is given priority to 10~40 g in this hospital, and compared to 2014, the average per patient dosage has a tendency to increase gradually; to supply blood volume and remedy hypoproteinemia after a surgical, to ameliorate the low blood albumin of tumor patients and to reduce the intracranial pressure of patients with cerebral blood flow perfusion are the first three reasons. There are 118 unreasonable patient records in the reviewed 233 patient records, Don’t GeLiLv reaches 50.64%.The reason for irrational drug-use involves the no determination of patients serum albumin values, lack of drug against, large dosage and contraindications in drug-using, etc. ConclusionHuman Albumin is widely used in this hospital involving patients of all ages, all hospital wards, various kinds of diseases; the number of patients, the average dosages and the number of drugs also showed an increasing trend, with the existence of chaos abuse problems; there exist some irrational drug-use, such as no determination of patients serum albumin values, lack of drug against, large dosage and contraindications in drug-using, etc..
