The Efficiency Research Of Township Hospitals Based On Data Envelopment Analysis And Tobit Model

Since from our country executes the new medical reform in 2009,the government increased the investment in rural medical and health resources, in order to deepen the medical and health system reform and accelerate reform of the rural grassroots medical and health institutions. However, the efficiency level of the rural grassroots medical institutions is still low in our country, the rural residents of health service demand have failed to get the meet. As the main part of rural grassroots medical and health services, the township hospitals plays an important role in improving the level of rural grassroots medical and health services and guaranting for rural residents health. For the reality of high quality medical and health resources currently to the city too much, how to optimize the structure of rural medical and health resources and develop rural medical and health services become an important way to deepen reform of the rural medical and health system and meet the demand of rural residents’ basic medical and health services. Thus, as around the establishment of the rural medical and health service system, the efficiency problem about the township hospitals gets more and more attention,to analysis and evaluate township hospitals levels of efficiency and its influencing factors, and strive to improve its efficiency has a very important and practical significance.This paper uses DEA-Tobit model and taking 25 township hospitals in Hunat. for instance, to evaluate and analysis the efficiency of township hospitals and its influencing factors. The DEA evaluation result is:involved in the 25 township hospitals, there are 9 township hospitals overall effective and 16 township hospitals not, DEA effective rate was 36%. Pure technical efficiency has 11 township hospitals effectively and 14 non effective, pure technical efficiency was 44%. Scale efficiency has 9 township hospitals effectively, invalid for 16, scale efficiency was 36%. Tobit regression results showed as follows:in eight of the endogenous variable into the model, the proportion of senior technical staff, the proportion of outpatient service in hospital and pharmacy income proportion have no statistical significance to the efficiency of township hospitals, but the other five variables have statistical significance to the efficiency of township hospitals,there are health technical personnel number, per capita outpatient expenses, the average length of stay for disch- arge patients, rate of bed utilization,the daily average rate of outpatient visits per doctor, while the health technical personnel number and the average length of stay for discharge patients are negatively correlated with the efficiency of township hospitals, daily outpatient expenses, sickbed utilization rate and per capita physician visits were positively correlated with the efficiency of township hospitals. For five exogenous variables, the distance away from the county seat,resident population and the number of village clinics had no significant effect on the efficiency of township hospitals.In the 10% level, rural per capita annual fiscal revenue was negatively correlated with the efficiency of township hospitals, new rural cooperative medical care system covering level was positively correlated with the efficiency of township hospitals.The conclusion:under the existing investment scale, the output of some township hospitals didn’t get maximize, not only the inadequate investment restrict the development of the township hospitals but also the medical and health resources are waste and production capacity is insufficient. The construction of township hospitals relies on increasing investment, at the same time still need to pay attention to strengthening the connotation construction and talent team construction, implementating regional health planning rationally, optimizing the structure of the medical and health resources distribution, and improving the utilization efficiency of all kinds of resources. To improve the efficiency of township hospitals should also expand the public health service, and perfect the new rural cooperative medical system, in order to meet the demand of rural residents medical and health services.
