Study On The Status And Effect Of Antiviral Treatment For AIDS Patients In Dezhou City

BackgroundAIDS is caused by the AIDS virus(HIV)infection of infectious diseases,currently no effective vaccine prevention,the case fatality rate is high,AIDS has seriously affected the human economy and social development,which has become a critical public health and social problem.HIV virus is highly variable,high mortality,and there is no effective-prevention vaccine to avoid infection.With the wide use of high efficiency retroviral therapy,AIDS becomes a chronic control disease from a lethal disease.HAART can effectively inhibit the replication of HIV in the human body,improve the level of human CD4 count,reconstruct the immune function,delay the course of progress and reduce mortality.With the passage of time,the countries pay more and more attention to HIV/AIDS prevention and control work,free antiretroviral treatment into the standards of the state to relax for many times,causing AIDS patients receiving HAART has skyrocketed.This paper,through the understanding of the current antiviral therapy,evaluates the therapeutic effect,analyzes the influencing factors,studies the prevention and control measures,which provides a scientific basis for more targeted,scientific,normative antiviral treatment measures at the same time.Research PurposesThis paper aims to investigate the status of antiviral therapy in AIDS patients in Dezhou City,evaluate the effect of antiviral therapy for AIDS,analyze the influencing factors,to study the countermeasures,and provide scientific basis for future AIDS antiviral therapy.Objects and MethodsBy using the convenient sampling,the paper conducts a questionnaire survey of HIV/AIDS patients who are currently receiving antiviral treatment,to understand the status of AIDS antiviral therapy.A retrospective survey is conducted to collect data on antiviral therapy in HIV/AIDS patients who receive HAART for the first time,including demographic characteristics,baseline survey data,previous follow-up status,current outcome and so on.The collected data of the treatment are analyzed statistically to understand the effects of immune reconstruction,HIV virus suppression,survival time,etc.,and to evaluate the effect of antiviral therapy.The data of the on-site questionnaire are recorded by EPIDATA3.1 software.The collected AIDS virus antiviral data are collected by Excel software to create investigation database.And then the data are analyzed by SPSS 19.0.The selection of the logistic regression analysis affects the patient’s compliance,the choice of antiviral treatment institutions and the reconstruction of immune function.Hazard regression model of single factor and multivariate Cox proportional is used to compare survival rates.Results1.In the study,distributed questionnaires of antiviral therapy prevalence survey were counted to 273 copies,of which 264 valid questionnaires were collected(96.73%);306 cases were included in the evaluation of antiviral treatment effect.2.The status survey of AIDS antiviral treatment:the disease control agencies take the main body of the antiviral treatment in Dezhou City;the basic knowledge awareness rate of AIDS prevention and treatment is 97.35%(257/264),and the AIDS-related knowledge awareness rate is 65.53%(173/264);the results show that gender,marital status,occupational and social organization’s follow-up intervention are the influencing factors;228 cases(86.36%)patients think that the treatment effect is very good,and 95.45%(252/264)think they can cope with antiviral therapy;94.32%(249/264)of patients think they can adhere to the future antiviral therapy;the drug effective compliance ratio of the study object is 96.97%(256/264);the analysis shows that "self evaluation of therapeutic effect" and "attitudes toward future treatment" are factors leading to the occurrence of missed service.81.44%(215/264)of HIV/AIDS patients choose the future HIV antiviral treatment by the disease control agency,and 18.56%(49/264)choose medical institutions.3.The evaluation of antiviral therapy for AIDS:the initial treatment of using first-line scheme by the study object accounts for 91.50%(280/306);there are 80 cases(26.14%)HIV/AIDS patients who take cotrimoxazole prophylaxis and treatment of opportunistic infections;72 cases(23.53%)have the drug toxicity;52 cases(16.99%)replace the drugs,whose reasons include first-line drug treatment failure,toxic side effects and so on;11 patients on HAART withdraw from the treatment sequence,whose reasons include drug side effects,loss to follow-up,drug interactions;the most recent test results show that 203 cases(72.50%)patients achieve a good immune reconstitution,and long analysis show that the treatment Logistic,hepatitis B infection,baseline CD4 count are the immune reconstitution factors;the complete inhibition rate of 12-month treatment is 90.19%(193/214),the virus resistance is 6.16%(13/211);after receiving HAART,the survival rates of 1,3,12,18 months are 97.02%,96.01%,94.86%,93.63%,and five-year survival rate is 91.73%;the cumulative survival rate over 50 years old is 76.60%,and the survival rates with antiviral treatment for 3 years are 86.67%,82.98%,76.60%,lower than the other groups。Multivariate Cox analysis show that the age and baseline CD4+T lymphocyte count are factors in the survival time of patients.Conclusions1.AIDS anti-virus treatment in Dezhou City has achieved remarkable results.HAART has effectively improved the immune function of HIV/AIDS patients,inhibited the virus replication,improved the quality of life and survival of HIV/AIDS patients,and prolonged the lives of patients.Most patients can consciously cooperate with antiviral treatment services,medication compliance,and can give correct and just evaluation to antiviral treatment.2.The antiviral treatment model with the disease control mechanism as the main body in Dezhou City has been unable to meet the current needs of AIDS antiviral therapy,CDC HIV/AIDS antiviral treatment because of the lack of systematic and comprehensive training and medical qualifications,unable to deal effectively with patients with serious opportunistic infections,drug side effects,etc.,can’t make a thorough assessment of the patient condition exists serious medical risk and liability risks.The reconstruction of immune function is not ideal.The baseline CD4 for counting low population,elderly patients need more attention.Some patients have psychological resistance on the current antiviral treatment services.Self-treatment effect evaluation is not high,medication compliance is poor,and most patients still hope that the disease control agencies are responsible for anti-virus treatment services.3.It is time to change the current antiviral treatment mode,to implement the"informed consent" antiviral treatment principles,to meet the patient’s needs such as"professional","privacy","convenient" and other anti-virus treatment needs,to focus on optimizing resources,to simplify process,to strengthen medication compliance education,to give full play to the role of social organization,to explore patients with subjective initiative.At the same time,pay attention to protecting the privacy of patients,being concerned about the psychological changes of patients and providing more high-quality professional medical services.
