| [Objective]In this study,a LACE based risk prediction model was established for patients with acute myocardial infarction,and to evaluate its clinical application.[Method]In this study,by introducing the LACE risk prediction model,based on the risk prediction model,the hospital discharge intervention plan for acute myocardial infarction was established.A convenient sampling method was used to select 120 patients with AMI in Affiliated Hospital of Jiangsu University from November 2015 to May 2016.120 cases of AMI patients admitted by LACE risk scores before discharge application randomly stratified randomization,intervention group and control group with 60 cases in each group,the control group used the traditional management mode of the development of AMI discharge planning intervention group of AMI patients during the evaluation of various health problems of patients and their families and need hospitalization,provide health education and nursing measures of fit,the LACE index scores at discharge,according to the score provided two primary prevention of coronary heart disease related to the contents of the guidance.The general data of patients were collected,and 6 months after discharge,the quality of life of patients,the management of coronary heart disease,cardiac function and the rate of unplanned return were collected.[Results]107 patients were successfully followed up,the intervention group was given to the control group(n = 54)and the control group(n = 53).The patients in the intervention group plans to return to the non rate and the incidence of cardiac events was lower than the control group(P<0.05).The patients in the intervention group improved heart function than the control group(P<0.05).[Conclusion]considering risk factors in AMI patients,the established prediction model of discharge intervention plan risk based on LACE index,can reduce the unplanned readmission rate,improve the prognosis of patients with AMI,improve medical safety and quality management. |
