The Research Of Transitional Nursing Mode Had The Effects On Quality Of Life For The Sequelae Of Children With Severe Viral Encephalitis

Objective: To base on the survey for the sequelae of children with severe viral encephalitis of parents’ transitional care of needs,it is finally established transitional nursing mode for sequelae of SVE children,and to verify its effectiveness.Methods: Experimental study and nonsychronous control.From July to October 2015 through self-designed questionnaire,extracted of the discharge of parents with sequelae children of 126 cases with severe viral encephalitis in Guizhou Medical University Affiliated Hospital、The People’s Hospital of Guizhou Province、Children’s Hospital of Guizhou Province with the method of convenient sampling.From October to December 2015,Delphi technique was used two-round consultation of 22 experts of 6 hospitals and 1 nursing school in Guizhou province.According to the results of consultation,it was finally established transitional nursing mode for sequelae of SVE children.A total of 44 sequelae of SVE discharge children and parents were chosen between July and December 2015 as control group and received normal nursing after discharge in Guizhou Medical University Affiliated Hospital.A total of 44 discharge SVE children and parents were chosen between January and June 2016 as experiment group and provided transitional care mode to them in Guizhou Medical University Affiliated Hospital.PedsQLTM4.0 scale was used to assess sequelae of SVE children quality of life discharged and left hospital in 1th、3th,and compared each evaluation indicators to the experiment group,PedsQLTM4.0 scale was used to assess sequelae of SVE children quality of life discharged and left hospital in 1th、3th,and compared each evaluation indicators to the control group and the experiment group conducted children family care knowledge questionnaire score discharged、leaving hospital in3 th.T-test,Chi-square test,Mann-Whitney U,Pared-samples T test and Repeated measures analysis of variance were used to analyze the data with SPSS 22.0.Results:(1)Through surveyed the discharge of 126 SVE parents’ needs of continuing care,94% parents wanted to get transitional nursing,in 21 items of transitional care content requirements,the results showed that the needs of the parents were :100.00% parents had strong demands in the spirit of the psychological consultation,about 96.04% parents had aspects of needs in movement dysfunction complications,in addition,parents also had needs in social support、life care、health education nursing and care ability.The top three model of demand respectively was telephone and message was 88.12%、outpatient appointment was 83.17%、network communication was 76.24%,and home visit needed minimum.(2)After the two-round consultation,the active coefficent of experts was 100%,and the degree of specialist authority(Cr)was 0.87,the coordination indexes(W)at every level were respectively 0.324 and 0.516,statistically significant had difference α=0.05.It was finally determined that in the continuing plan,4 first-class indexes and 25second-class indexes were included.The primary index entry score mean ± standard deviation is 3.93±0.86 ~ 4.87±0.35;coefficient of variation(CV)was 0.07 ~ 0.20;full mark ratio is 46.67% ~ 86.67%,the selectivity is 73.33% ~ 100.00%.Secondary index entry score mean± standard deviation is 4.21±0.87~4.94±0.27;coefficient of variation(CV)was 0.07 ~ 0.22;full mark ratio is 40.00%~ 93.33%,the selectivity is73.33% ~ 100.00%.(3)Implementing transitional nursing mode,bases data of two groups with children discharged had no statistical differences(P>0.05).The dimensionalities of Peds QLTM4.0 score of 5 to 7 years old and 8 to 12 years old children discharged had no statistical differences(P>0.05).Peds QLTM4.0 score of5 to 7 years old and 8 to 12 years old children of two groups discharged、after leaving hospital in 1th and 3th had gradually increased and had statistical differences(P<0.05),the experimental group scored higher than the control group(P<0.05).The knowledge score of experiment group after leaving hospital in 3th statistically higher than before discharged(P<0.05).Conclusion:(1)Transitional care needs of the investigation is the basis for establishing transitional caremode.(2)Transitional nursing mode constructed is innovative and practicable,which may provide basis references for pediatric nursing.(3)The evaluation of life quality reflects the implement effect of nursing mode with SVE sequelae children,and enhances the master degree of disease knowledge of parents,and be worth popularizing in clinical work.
