The Research On Risk Of Home Safety And Nursing Needs Of Elderly Patients With Stroke

BackgroundDue to the high incidence,high mortality,high disability rate,and a longer recovery period,patients with stroke have different degrees of dysfunction of visual,the older the morbidity and the higher the mortality rate.In recent years,it has become a major public issue that seriously endangers the health and life quality of the aged people.Due to economic conditions and the impact of traditional factors,the majority of patients select home-cared which there are many security risks existed,so it brings trouble to the stroke family in the recovery period.ObjectiveThe aim of this study was to investigate the current status and main influencing factors of the elderly patients with stroke in the home environment,and to explore the need of safe nursing in the elderly patients with stroke at the early stage of hospital discharge and rehabilitation.And to improve the rehabilitation of patients with quality of life to provide a theoretical basis.Methods1.Subject.The research select three-hundred stroke patients those who met inclusion criteria and exclusion criteria discharged from three Class-three hospitals in one city within one month as research object.2.The investigation content include the general situation of elderly stroke patients,ADL,home safety hazards and nursing needs,the main research tools used were(1)General Information Questionnaire,including age,medical expenses and living forms of payment of nine projects.(2)Modified Barthel index scale,measure the ADL,including eating,bathing,and dressing 10 items.Grading: 0 grade capacity intact,score of 100 points;a mild deletion,score 65 to 95 minutes;2 moderate deletion,score in 45 to 60 minutes;3severe deletion,score ≤ 40 points.The Cronbach’s alpha coefficient was 0.815 after a small sample survey.(3)Questionnaire survey on the safety of elderly patients with stroke is a self-designed questionnaire with 4 dimensions and 25 items including safety of drug use,home environment safety,food safety and safety of rehabilitation activities.After a small sample survey,the test-retest reliability of the questionnaire was 0.819,and Cronbach’salpha coefficient was 0.831.(4)Questionnaire of home nursing and nursing needs of elderly stroke patients: The self-designed questionnaires were reviewed by literature and experts,and finally included 16 items.After a small sample survey,the test-retest reliability is0.863 and the Cronbach’s α coefficient is 0.827.Through data collection and data processing,the SPSS17.0 software is used to analyze the data of the original data.Results1.Activities of daily living.There are significant differences in the safety hidden danger(F=6.021,P<0.01)and nursing needs(F=3.951,P<0.01)of the elderly with different ADL levels.2.The total score of home safety risk for elderly patients with stroke was(68.880 ±9.548),and the standard is 68.880.Dose,side effects and contraindication score the lowest(1.920 ± 0.689).There were statistically significant differences in the safety hidden danger between family members,family income,occupation,and whether chronic diseases were combined,P <0.01.3.The total score of nursing needs of elderly patients with cerebral apoplexy is(43.800 ± 5.653),and the standard is 68.438.Emergency first aid knowledge project scores the highest(3.840 ± 0.367).There are significant differences in the gender,marital status,educational level,family income,living form and other characteristics of the elderly patients with stroke,P < 0.01.4.Whether the combination of chronic disease,the degree of education,family income,occupation of before retirement,living form and other 5 variables entered the regression equation(P <0.001)are the main influencing factors of the elderly stroke patients in home security risk.Educational level,marital status,payment method and other8 variables entered the regression equation(P <0.001)are the main influencing factors of home care needs of elderly patients with stroke.Conclusions1.Elderly patients with stroke risk at a moderate level,the need to increase the home of elderly patients with stroke safety concerns.Old age,low income,low education,patients with chronic diseases greater security risk.2.The overall situation of nursing needs survey score the middle,home care needs more security.Male,divorced or widowed,higher educational level and higher income,ADL of the patients with nursing need more home security.
