| Part 1 Reference Intervals for Common Thyroid Function Tests in Pregnant Women in FuyangObjective To investigate reference intervals for common thyroid function tests in pregnant women in Fuyang.Methods The study was surveyed in population taking their prenatal care at the No.1 People’s Hospital of Fuyang between Apr1 2015~Sep30 2015.The clinical history of all the pregnant women enrolled and their serum TSH、FT4、FT3、TPOAb、TGAb were recorded and analysised according to the National Academy of Clinical Biochemisty.2041 cases were included.The 2.5th and 97.5th percentiles of each stage were calculated.Results The level of TSH in pregancy women in Fuyang were:T1(0.02~3.53)m IU/L;T2(0.13~3.73)m IU/L;T3(0.18~5.23)m IU/L;the median level of TSH was gradually increase and the P97.5 of each trimester was higher than the recommended value in 2011 ATA guideline.The level of FT4 in pregancy women in Fuyang were:T1(0.87~1.49)ng/dl;T2(0.77~1.28)ng/dl;T3(0.74~1.39)ng/dl;The level of FT3 in pregancy women in Fuyang were:T1(2.21 ~3.99)pg/ml;T2(2.10~3.76)pg/ml;T3(2.05~3.70)pg/ml.Conclusions The reference value of TSH in pregant women in Fuyang is different form the recommended Value in 2011 ATA guideline.Part 2 Study of Two Different Criteria and Screening strategies for Subclinical Hypothyroidism in Pregnant WomenObjective To investigate Two Different Criteria and Screening strategies for Subclinical Hypothyroidism in Pregnant Women.Methods The study was surveyed in population taking their prenatal care at the No.1 People’s Hospital of Fuyang.There were 2421 pregnant women enrolled in this study.The clinical history of all these women was collected and they were divided into high-risk and low-risk groups.Their serum TSH、FT4、FT3、TPOAb、TGAb were recorded and analyzed according to the following two criteria:1)foreign criterion: according to 2011 ATA guideline;2)national criterion:according to 2012 Chinese guidelines for the diagnosis and treatment of thyroid disease during pregnancy and postpartum.Morbidity rates of subclinical Hypothyroidism were compared between different diagnosis and groups.Results Morbidity rates of subclinical Hypothyroidism with national criterion was 3.8%,lower than 18.7% with foreign criterion(P<0.001);In high risk group,morbidity rates of subclinical Hypothyroidism with national criterion was 5.5%,with foreign criterion was 24.1%,which was higher than in low risk group with no statistical significance(P>0.05).According to foreign criterion,if we had only tested High-risk groups,misdiagnosis rate of pregnancy with subclinical Hypothyroidism would be 82.7%.According to national criterion,if we had only tested High-risk groups,misdiagnosis rate of pregnancy with subclinical Hypothyroidism would be 84.5%.Conclusions In diagnosis subclinical Hypothyroidism in pregnant women,morbidity rate with foreign criterion is higher than with national criterion.If we only test high-risk groups,misdiagnosis rate of pregnancy with subclinical Hypothyroidism may be high. |
