| Introduction:Kawasaki disease,mainly causes acute necrosis media-small vasculitis of unknown etiology,that mostly occurs in Asian counties as Japan and Korean children from 6 months to 5 years of age.The disease predominantly involves the coronary arteries,causing various cardiac complications,which may be fatal and life-threatening.Incomplete Kawasaki disease(IKD),also called atypical Kawasaki disease(AKD),is very hard for the early diagnosis,particularly difficult to diagnosis if the symptoms tend to present for a short time or at different times.Some retrospective studies indicated that IKD/AKD are usually linked to higher incidence of cardiac events than the complete Kawasaki disease.Here,we report a rare case of the complication associated with Kawasaki disease,the patient is a 14 year-old Chinese girl.Case report:A 14 year-old Chinese girl presented sudden cardiopulmonary arrest during the exercise class.A series of tests as echocardiography,coronary computed tomography angiography(CTA)and coronary arteriography indicated coronary abnormalities,with her past medical history of IKD,the ultimate diagnosis was quite clear-the complication associated with Kawasaki disease,giant coronary artery aneurysm with thrombogenesis,and acute myocardial infraction.Subsequently coronary artery bypass graft(CABG)was performed successfully and combined antiplatelet therapy was discharged.Followed up for 2 years,the girl’s daily life was not affected,and no abnormalities was found when underwent the reexamination of coronary CTA.Conclusion:This case report of the complication associate with Kawasaki disease indicates the vital importance of IKD early diagnosis,especially for the pediatricians,who should be alert for IKD in case that the delayed treatment caused more fetal long-term complications.Also,for young adults with coronary artery ectasia,it is better for clinicians,in consideration of Kawasaki disease in the past medical history,to carefully assess the surgical indication for IVIG and treat actively to improve prognosis. |
