Investigation And Analysis Of Etiology And Syndrome Of 109 Patients With Osteonecrosis Of The Femoral Head

Objective:To investigate the etiology and syndromes of patients with femoral head necrosis in our hospital and to analyze its characteristics,to explore the characteristics of femoral head necrosis,statistics and analysis of TCM syndromes and etiology,that the clinical prevention of femoral head necrosis,diagnosis and Treatment for reference.Method:First,we conducted the initial screening of records from the Department of medical records information access system,and looked over the patients with femoral head necrosis from January 2016 to December 2016 in Fuzhou Second Hospital。Using "drug discharge diagnosis of femoral head necrosis,traumatic femoral head necrosis,alcohol induced osteonecrosis of the femoral head,femoral head aseptic necrosis" as keyword search,and limited the time form the January 1,2016 to December 31,2016,according to this we acquied the list of patients with femoral head necrosis in Fuzhou Second Hospital during this period,from the inpatient information table and medical records access to medical records of patients,and then by the specialized personnel by artificial screening all patients of femoral head necrosis the basic information and record basic information,including name,gender,age,home address,the main diagnosis,and the main TCM syndrome,tongue and pulse,the required information and data were entried statistical information table.Then the data were analyzed by using statistical software SPSS20.0 to analyze the relationship between age,sex,incidence of disease,pathogenesis,TCM syndromes and the factors of the subjects.Result:(1)109 cases were included in the study,the average age of male patients was(48.80 + 13.4)years;the average age of female patients was(55.78 + 12.99)years old,male patients than in female patients with mean age 6.97 years younger,the age of onset of male and female patients with significant difference(t=-2.579,P=0.011);40～59 years of age patients occupy the largest proportion,46.79%;male and female patients is 2.11:1.(2)of all the patients with ONFH,the bone necrosis caused by traumatic factors was the first,followed by the hormone,alcoholic and idiopathic.The different gender of ONFH patients,its etiology is not the same,there are obvious differences(χ2=12.05,P=0.006)in different age,traumatic factors are occupied first place,at the age of 60 years in the above factors occupy a significant proportion,up to 74.19%;analyze the constituent ratio of patients with different ages the cause,the result show that there is obvious differences(χ2=10.703,P=0.013);analysis of single bilateral etiology than,the result show that there is obvious difference(χ2=20.112,P=0.000),a bilateral unilateral disease is more common,and the single side caused by traumatic disease,multiple bilateral disease caused by hormone.(3)109 cases of qi stagnation and blood stasis syndrome is the most common,followed by liver and kidney deficiency,phlegm and blood stasis syndrome and meridian obstruction syndrome;syndrome of male and female patients in the first place are qi stagnation and blood stasis syndrome,but the syndrome constituent ratio was significantly different(x2=9.389,P=0.023),men with qi stagnation blood stasis,phlegm and blood stasis syndrome is the most common,and women with qi stagnation and blood stasis syndrome of deficiency of liver and kidney is the most common,phlegm and blood stasis is the most rare;analysis of the constituent ratio of patients of different age syndromes,the results showed that there was no significant difference(x2=8.219,P=0.215),which indicates that the age of the distribution of TCM Syndromes of roughly the same;an analysis of different factors in patients with ONFH syndrome.The results show significant difference(χ2=64.271,P=0.000),traumatic ONFH patients with qi stagnation and blood stasis is the most The main syndromes of ONFH patients;hormone deficiency of the liver and kidney is the main syndrome;alcoholic patients with ONFH syndrome of phlegm and blood stasis is the main syndrome.Conclusion:Traumatic avascular necrosis of the femoral head has the highest proportion,40～59 years old is the peak incidence of age,the incidence of male was more than female,the age of onset of male young;the prevalence of unilateral bilateral is more common,and caused many traumatic unilateral,bilateral disease caused by multiple hormones;Qi stagnation and blood stasis with the highest proportion of trauma factors caused by qi stagnation and blood stasis,deficiency of liver and kidney syndrome caused by hormonal factors,alcohol caused by phlegm and blood stasis syndrome.
