ObjectiveA preliminary understanding of the clinical epidemiological characteristics of respiratory tract virus infection in elderly patients with pneumonia,explore the distribution characteristics of Traditional Chinese Medicine(TCM)syndromes and pathogenesis evolution law,to provide ideas for clinical syndrome differentiation and treatment.MethodsThis study used a cross-sectional survey,epidemiological investigation and other methods,The Guangdong Province Hospital of Chinese Medicine(Observation Area,Comprehensive Area,the Third comprehensive Department)treatment of pneumonia in the elderly(over 60 years)patients were tested for respiratory viruses.Collect,organize and analyze general information,Western medicine auxiliary examination,TCM syndrome information about patients.ResultsDemographic Characteristics:In this study,85 patients with respiratory tract infection were collected,among them,50 were male and female were 35,there was no significant difference in gender.The minimum age of the patients was 60 years,with a maximum age of 97 years,with an average age of 78.41±8.58 years;there were 56 cases in the 75 years of age or older patients,accounted for 65.88%.The first five patients with pneumonia in elderly patients were hypertension,coronary heart disease,sequelae of cerebral infarction,type Ⅱ diabetes mellitus,chronic heart failure,however,patients with malignant tumor and immune suppression also occupy a certain proportion.Clinical Data of Western Medicine:The patient’s white blood cells and neutrophil count normal or can be slightly elevated,decreased lymphocyte count,some cases have reduced the trend of platelet count;oxygen partial pressure and oxygenation index have different degrees of decline;electrolyte metabolic disorders are frequent,liver and kidney dysfunction is common;troponin and myocardial enzymes have increased,there may be viral myocardial injury situation;coagulation index variation significantly,but need to further assess the relevance of the severity of infection;serum lactate concentration can be used as an early indicator to assess the severity of the disease and predict the development of the disease.Electrolyte metabolic disorders,renal dysfunction,respiratory failure,sepsis is a common complication of elderly patients with pneumonia,severe cases can be acute heart failure,acute renal failure,what is more,leading to septic shock,acute respiratory distress syndrome,infectious multiple organ dysfunction syndrome.Viral Etiology:All the patients included in this study were community acute onset,denial of contact with live poultry or febrile patients,epidemiology is more representative.Virus detection specimens with serum based(87.06%),the other is throat swab(12.940%).The virus detection mechanism is mainly based on the Laboratory of Guangdong Province Hospital of Chinese Medical,supplemented by Guangzhou Medical University(National Key Laboratory of respiratory diseases).The main types of respiratory tract infection were influenza virus and parainfluenza virus,influenza A virus in 34 cases(40%),influenza B virus in 24 cases(28.24%),parainfluenza virus 39 cases(45.88%).Distribution of TCM Syndromes:Fever,cough,expectoration,shortness of breath,chest pain and other lung disease manifestations are common complaints of visiting patients,most of the patients with mental fatigue,limb weakness,aversion to cold(or shivering),loss of appetite,upset insomnia and other symptoms.Dark red tongue,pale dark tongue,red tongue is the main color of tongue;tongue coating to yellow moss,greasy fur,white moss mainly;string pulse,pulse number,pulse,thready pulse,floating pulse are the main pulse.The lesions of this disease in the lung,often involving the spleen,kidney,heart,brain(God);the disease is mainly divided by weifen,followed by qifen,disease involving both weifen and qifen,sporadically appear blood,nutrient and blood;the syndrome type of disease is mainly qi deficiency,blood stasis,phlegm-heat,yin deficiency,damp-heat,wind-heat,both yang deficiency,retained fluid,phlegm,dampness and blood stasis,heat,wind-cold.The incidence of respiratory viral pneumonia in elderly patients,it is due to internal injury and diseases caused by exogenous pathogenic factor,zang and fu internal injury,internal organs,qi,blood,yin and yang deficiency is the main cause,lung defensive qi is not solid,affected by six exogenous pathogenic factors and pestilence gas is an important condition.ConclusionPneumonia is a common and frequently occurring disease in elderly patients,it is also the cause of acute exacerbation of a variety of basic diseases,especially chronic respiratory diseases in elderly patients.The rapid onset,rapid change,more complications,severe illness,seriously affects the quality of life and life safety.Respiratory tract virus is an important causative agent of pneumonia in elderly patients,especially worthy of attention in clinical diagnosis and treatment.The respiratory virus infection of pneumonia in the elderly,it is exogenous and internal injuries for the disease.The disease is located in the lung,lesions often involved in the spleen,kidney,heart,brain(God);deficiency of vial qi is the root cause of this disease,excess of pathogen is symptoms of this disease;core pathogenesis is excess of pathogen and vital essence deficiency,and through the whole process of the disease.
