| Background Dermoscopy digital image analysis technology is a kind of noninvasive examination method,it can not only provide the light source,magnify the skin size,but also through the horny layer,showed epidermis and the dermis and shallow,to observe the microscopic structure,such as pigment and vascular structures that the naked eye can not see [1].In recent years,the dermoscopy has been used in skin diseases,such as skin tumor,vascular disease,bacteria and virus infection and inflammatory skin diseases,etc.[2],but there is more about decreased pigment skin disease for further study.This article observe 1454 cases of common pigment decreased skin disease,Vitiligo,Pityriasia alba,Nevus depigmented,Postinflammatory hypomelanosis,Progressive macular hypomelanosis,Senile Leukoderma,Amelanotic nevus and HMF clinical dermoscopy image characteristics of mole analysis technology,to provide useful clinical diagnosis.Objective Through large sample image observational research,the establishment of clinical images charaterized analytical techniques by dermoscopy for Vitiligo,Pityriasia alba,Nevus depigmented,Postinflammatory hypomelanosis,Progressive macular hypomelanosis,Senile Leukoderma,Amelanotic nevus and HMF.Methods Respectively to observe the skin lesions of 1454 patients with decreased pigment disorder,including 837 cases of vitiligo,suspected Advanced stage in 322 cases,stable in 283 cases,232 cases of convalescence;suspected pityriasis alba in 147 cases,137 cases of nevus depigmentosus,inflammation after melanocytes to reduce disease in 133 cases of macular pigment,reduce disease 82 cases of senile leukoplakia in 58 cases,45 cases of anemia nevus,hypopigmented mycosis fungoides in 15 cases.The changes of the characteristic images of various hypopigmentation diseases such as background color,vascular morphology,boundary condition,abnormal hair and hair follicles,and pigmentation of the skin were observed and analyzed by means of a microscope.Results Progress vitiligo: white background,fuzzy boundaries,follicle week pigment residues surrounding irregular spherical,irregular hypopigmentation spots,patchy pigment residues.Stable vitiligo: porcelain white background,clear boundaries,follicle-week residual pigment,skin pigmentation deepened week.Recovery of vitiligo: a clear boundary,follicle-week residual pigment,skin pigmentation deepened week,the pigment is easy to see the island.The pigment residue for the characteristic of vitiligo skin hair follicle weeks mirror image performance,porcelain white background for vitiligo is cutaneous mirror image performance characteristic,stability for recovery is characteristic of vitiligo skin pigment island mirror image performance.Pityriasia alba: yellowish white background,surface small scaly or flaky white bran,fuzzy boundaries.The gray scales of chaff shape is white pityriasis skin characteristic of mirror image,to distinguish it from other diseases.Nevus depigmented: piebald like background,borders blur,patchy pigment residues.One spotted color background is no cutaneous mirror image characteristic manifestation of pigmented nevus.Postinflammatory hypomelanosis: yellow-white background,fuzzy boundaries.Progressive macular hypomelanosis: light white background,fuzzy boundaries.Senile Leukoderma: white background,clear boundary.Inflammation after sexual patchy pigment melanin reduce disease,reduce disease and senile plaques characteristic without skin mirror image performance.Amelanotic nevus: yellow-white background,fuzzy boundaries,peripheral vascular distribution grid after friction.Friction in skin lesions of the edge of the normal skin see bulky clear vascular network distribution,and white spot not seen vascular structures is anaemic mole is characteristic skin mirror image performance.HMF: reticular,zebra-like or dotted depigmentation,and blood vessels distributed in punctiform,short linear,spermatozoa-like or stellate patterns,as a specific skin mirror image characteristics.Verification rate: differentiation between vitiligo and other hypochromic skin diseases,both skin and skin CT are specific.White chaff,colorless nevus,senile white spot,anemic nevus,two of which are identified by each other,the skin mirror is specific,and the skin CT cannot be identified.Progressive macular pigment reduce disease,inflammation,melanin after disease under the skin and skin CT has no specific performance,can be combined with clinical characteristics,Wood ’s lamp and so on diagnosis and differential diagnosis.The index of the image of the hypochrome image and pathological biopsy was 86.7%.Conclusion The clinical images charaterized analytical techniques by dermoscopy can be used as diagnosis and differential diagnosis of Vitiligo,Pityriasia alba,Nevus depigmented,Senile Leukoderma,Amelanotic nevus and HMF. |
