The Application Of Small Pupil Cataract Phacoemulsification With Built-in Iris Dilator

Objective: Compare the effect of samll pupil phacoemulsification with OASIS built-in iris dilator and non pupillary dilation in the following aspects:the whole operation time,the effective phaco time(EPT),the postoperative aqueous flash,the corneal endothelial cell counts,the corneal edema,the best visual acuity,and the difficulty of the operation and evaluation of the patients’ satisfaction.Methods: 32 patients(eyes)with small pupil cataract(drug can not disperse the pupil)which was selected from December 2015 to December 2016 in our hospital is set as OASIS group,and the conventional surgery group select the retrospective clinical case of small pupil cataract(without pupil dilation)of 32 patients(eyes).OASIS group performed PHACO +IOL with built-in OASIS iris dilator;conventional surgery group do the same without pupillary dilation.The intraoperative data of the whole operation time and effective phaco time were compared.Postoperative changes of aqueous flare,corneal edema and corneal endothelial cell counts,the best corrected visual acuity,the difficulty of operation and the satisfaction of patients were compared in 30 days after operation.Results: For grade III,IV nuclear density,the whole operation time of two group was statistically significant difference(P=0.004,0.001),but not in grade II nuclear density(P=0.463).For grade II,III,IV nuclear density,the EPT of OASIS group were all significantly lower than the conventional surgery group,the difference was statistically significant(P=0.003,0.000,0.000).The aqueous flash at 1d after surgery was statistically significant difference(P =0.018),It disappeared after 1w.The corneal edema at 1d,7d after surgery was statistically significant difference(P= 0.001,0.012).The diference of corneal endothelial cell c ounts at 7d,30 d was statistically significant(P=0.001,0.04).The best corrected visual acuity was statistically significant difference at 1d(P =0.001),but not at 7d,30d(P=0.053,1.000).There was significant difference in operation difficulty a nd satisfaction between the two groups(P=0.000,0.047).Conclusion: Compared with non pupil dilation,OASIS built-in iris dilator used in small pupil cataract phacoemulsification has several advantages: reduced operation difficulties,shorten phaco time,less tissue damage,higher patient satisfaction.It is safe and effective for clinical application.
