| Objective: To investigate the incidence of clopidogrel resistance(CR)and adverse events after PCI in patients with type 2 diabetes mellitus complicated with acute coronary syndrome(ACS)after taking anti-platelet drugs. Methods: from July 2015 to July 2016 in our hospital undergoing percutaneous coronary intervention(PCI)in 107 cases of ACS patients according to whether patients with type 2 diabetes were randomly divided into diabetes group and non diabetes group by Thrombelastogram(TEG)were measured in two groups of patients taking aspirin and clopidogrel for 24 hours after the ADP pathway of platelet inhibition ratio.Calculate the number and Gensini of vascular lesions score according to the results of coronary angiography;subgroup analysis according to the group of diabetic patients admitted to normal or high glycosylated hemoglobin;6 patients after PCI months follow-up,the incidence of comparison between CR group and non CR group of patients with adverse cardiovascular events. In addition,CR group of patients with diabetes and non diabetes subgroup analysis, the incidence of adverse cardiovascular events. Results: the patients with type 2 diabetes in 13 cases of ACS(38.2%)CR,non-diabetic patients 11 cases(15.1%)CR,the incidence of multivessel disease incidence and lesion complexity were higher than the non diabetes group of diabetic patients in the CR group, the difference was statistically significant(P<0.05).Diabetic subgroup analysis found: higher than normal group, the incidence of CR was higher in patients with glycosylated hemoglobin, and the difference was statistically significant(P<0.05).CR group compared with non CR group,a higher incidence of adverse cardiovascular events, the subgroup analysis found that CR group: Patients with diabetes compared with non diabetic patients, the occurrence rate of adverse cardiovascular events is higher, the difference was statistically significant(P<0.05).Conclusion: CR and multivessel coronary disease incidence, severity is more complicated with diabetes ACS patients more easily, antiplatelet drugs in patients with diabetes and poor inhibitory effect in patients with recent poor glycemic control related. The incidence of adverse cardiovascular events in patients with CR after PCI was higher, and the prognosis was poor, and the incidence of cardiovascular disease in CR patients with diabetes mellitus complicated with diabetes. |
