ObjectiveTo discuss the application value of PET-CT combined with serum carcino-embryonic-antigen(CEA),neuron specific enzyme(NSE),cytokeratin 19 fragment(CYFRA21-1)and squamous cell antigen(SCCA)in peripheral lung cancer.MethodsCollected in our hospital in January 2014 December-2016 initial suspected peripheral lung cancer,lung cancer and parallel PET-CT four tumor markers(carcino-embryonic-antigen,neuron specific enzyme,cytokeratin 19 fragment,squamous cell carcinoma antigen)examination.Retrospective analysis was performed.Cases were included in the standard for:1,All the cases were not treated before check;2,All cases were adopted different ways to achieve the exact pathological results;3,All the cases were examined by PET-CT and four lung cancer tumor marker examination.45 patients met the inclusion criteria.There were 29 males and 16 females,aged 34-86 years old.The average age of the patients were 56 years old.Among them 35 cases were malignant tumor,10 cases were benign lesions including:26 cases of adenocarcinoma,9 cases of squamous cell carcinoma.Benign lesions were 10 cases,including 3 cases of inflammatory granuloma,6 cases of inflammatory lesions,lease of bronchial cyst.Positive indexes of PET-CT were lesions with lobulation and other malignant features,SUVmax is more than or equal to 2.5.Lung cancer tumor markers include carcino-embryonic-antigen(CEA),neuron specific enzyme(NSE),cytokeratin 9 fragment(CYFRA21-1)and squamous cell carcinoma antigen(SCCA).Positive index were CEA>5.0ng/ml,NSE>18.3ng/ml,SCCA>2.5ng/ml,CYFRA21-1>7ng/ml.Check the consistency of PET-CT alone and pathological results by Kappa value judgment.Check the consistency of PET-CT combined with lung cancer four tumor marker examination and pathological results by Kappa value judgment.Compared the sensitivity,specificity of them by chi square test.Check tumor markers and different pathological types have statistical significance with t test.SUVmax values are statistically significant differences in different pathological types of t test.Check the application of SUVmax value and the size of lesion with Pearson method.ResultsCompared with PET-CT in combination with tumor marker of lung cancer examination and PET-CT examination alone,PET-CT combined with tumor marker of lung cancer in sensitivity,specificity and accuracy were higher than those in the PET-CT examination in all underwent PET-CT and tumor marker of lung cancer patients.There was significant difference between the sensitivity,specificity of difference with statistical significance.The use of PET-CT combined markers and pathological results are highly consistent and a separate PET-CT examination with the general consensus.Malignant lesions of SUVmax is greater than that of benign lesions.SUVmax of squamous cell carcinoma was significantly higher than that of adenocarcinoma.SUVmax of adenocarcinoma was higher than that of benign lesions and the difference was statistically significant;The size of lesions and the SUVmax had a significantly positive correlation with statistical significance.ConclusionPET-CT combined with serum carcino-embryonic-antigen(CEA),neuron specific enzyme(NSE),cytokeratin 19 fragment(CYFRA21-1)and squamous cell carcinoma antigen(SCCA)in peripheral lung cancer in sensitivity,specificity,accuracy is higher than that of the PET-CT test alone,which has important significance and unique clinical value in the diagnosis of peripheral lung cancer.
