| Objective:To discuss the influence of Urinary albumin to creatinine ration in H type hypertension and the difference in gender,then to prove the damage of Hyperhomocysteinemia to kidney and provide theoretical basis for individualized treatment with H type hypertension.Methods:A total of 863 patients were diagnosed hypertension at 5th Department of Cardiology in the Second Hospital of Dalian Medical University Hospital from December 2015 to December 2016.A11 subjects are excluded secondary hypertension,diabetes,with other diseases of urinary system such as acute or chronic nephritis,nephrotic syndrome,urinary infection,and acute or chronic renal failure,coronary heart disease,peripheral vascular disease,valvular heart disease,rheumatic heart disease,acute or chronic heart failure,congenital heart disease,sleep apnea hypopnea syndrome,pregnancy,malignant tumor,acute or chronic infection,、uarthritis,autoimmune diseases and the people who taken folic acid or vitamin B12,B6 recently and the women patients in period.Ultimately 333 patients enrolled this study,included 156 males and 177 females.The baseline information including age,gender,height,weight,calculation of body mass index(BMI),extraction of fasting venous blood to test homocysteine(Hcy),total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),ApoAl,ApoB,fasting blood glucose(FBG),D-Dimer,fibrinogen(Fig).high sensitive c-reactive protein(CRP),and urinary albumin to creatinine ratio(UACR).First,they are divided into two squads on the basis of homocysteine(Hcy)levels within or not the reference value.Then,Grouping was based on gender.In the group of men,they are divided into two squads on the basis of homocysteine(Hcy)levels within or not the reference value.In the group of women,they are the same to the group of men.Then comparing the urinary albumin to creatinine ratio level respectively.Last comparing the urinary albumin to creatinine ratio level with the male H hypertension group and the female H hypertension group.Results:1、The Hey level of the male group with hypertension was higher than that of the female group with hypertension,and the difference had statistical significance(P<0.05);The UACR level in the male group and the female group had no statistical difference(P>0.05).2.The UACR level of the H type hypertension group was higher than that of the non H type hypertension group,and the difference had statistical significance(P<0.05).3.In the male group,the UACR level of the H type hypertension group was higher than that of the non H type hypertension group,and the difference had statistical significance;(P<0.05);In the female group,the UACR level of the H type hypertension group and the non H type hypertension group had no statistical difference(P>0.05).4.The UACR level of the male H type hypertension group was higher than that of the female H type hypertension group,and the difference had statistical significance;(P<0.05).5.In the male group,Hey and age、height、TG、ApoA1、D-Dimer、Fib、UACR were positively correlated;The correlation index was small and had statistical significace with UACR(P<0.05);Hey and weight、TC、HDL-C、LDL-C、ApoB、CRP were negatively correlated;The correlation index was small and had no statistical significance(P>0.05).In the female group,Hey and age,、TG、CRP、UACR were positively correlated;The correlation index was small and had statistical significace with UACR(P<0.05);Hey and height、weight、TC、HDL-C、LDL-C、ApoA1、ApoB、D-Dimer、Fib were negatively correlated;The correlation index was small and had no statistical significance(P>0.05).6.In the male group,UACR and height、weight、TC、TG、LDL-C、ApoAl、ApoB、D-Dimer.CRP were positively correlated;The correlation index was small and had no statistical significance(P>0.05);UACR and age、HDL-C、Fib were negatively correlated;The correlation index was small and had no statistical significance(P>0.05).In the female group,UACR and age、height、TC、TG、HDL-C、LDL-C、ApoAl、ApoB、D-Dimer、Fib、CRP were positively correlated;The correlation index was small and had statistical significance with Fib、CRP(P<0.05);UACR and weight were negatively correlated;The correlation index was small and had no statistical significance(P>0.05).7.Binary Logistic regression analysis showed that LDL-C and Hcy were risk factors of early renal damage for men with hypertension,Fig and Hcy were risk factors of early renal damage for women with hypertension.Conclusion:1.The serum Hcy level of patients with primary hypertension had gender difference,men higher than women;The UACR level of people with essential hypertension had no gender difference.2.The UACR level of the H type hypertension group and non H type hypertension group had difference.3.The UACR level of the H type hypertension and non H type hypertension in men had difference;there was no difference in women.4.The UACR level of the H type hypertension in men group and the H type hypertension in women group had difference.5.LDL-C and Hcy were risk factors of early renal damage for men with hypertension,Fig and Hcy were risk factors of early renal damage for women with hypertension. |
