| Objective: In recent years,the relationship between doctors and patients has become increasingly tense.The number of medical disputes has increased greatly.The difficulty of handling has increased and the amount of compensation has become higher and higher.This has become a hot topic of social concern.Patients are increasingly dissatisfied with the hospital services and medical complaints are more and more.The principle of informed consent has been applied to physician-patient relationship and the clinical field.The maintenance of patients’ informed consent has been paid more and more attention by the public.Therefore,this paper is about the investigation for four hospitals’ medical complaints in 2012 ~ 2016 through screening out medical complaints caused by informed consent.The exposition is based on the specific and real data and the author analyze the reasons to get the conclusions.For the sake of finding the specific reasons for the occurrence of complaints and then drawing solutions,the author hope to make a modest effort for ameliorating today’s increasingly tense physician-patient relationship.Methods: 1.Empirical investigation.In the medical complaints reception department of four hospitals,the author selects representative medical complaints for on-site observation and audition.2.Statistical analysis.The complaints are sorted out by content to screen out medical complaints caused by informed consent to summarize and analyze.Excel is used to summarize and analyze the data.3.Literature research method.Through looking up the relevant information and literature,the medical complaints caused by informed consent are sorted out.4.Comparative research method.The four hospitals are sorted out by the hospital grade and then the author conducts comparative study for the corresponding hospital’s data.For the sake of protecting the hospitals’ privacy,the author uses the letters A,B,C,D as four hospitals’ code.A、B、C and D are all third-grade class-A hospitals,in which A and B indicate the two provincial hospitals,C and D indicate the two municipal hospitals.The author classifies the reasons caused by informed consent into the following aspects: the attitude of informing,the content of informing,(be)the object of informing,the way of informing,the confirmation of informing,the timeliness of informing,the disagree of informing(refused treatment).Because complaints caused by the content of informing account for the largest proportion compared to the other reasons,the author also classifies the contents of informing into four aspects: incompleted informing,exaggerated and improper informing,conceal the truth and medication,equipment and charges are not informed.Results: According to the data available,four hospitals received a total of 972 valid complaints from 2012 to 2015,with 198 cases of medical complaints caused by informed consent,accounting for the total amount of complaints 20.37%.Among the informed complaints,the proportion of complaints triggered by the content of informing is the largest part,accounting for 58.96%,ranked second is the confirmation of informing,accounting for 16.42%,ranked third is the time limit of informing,accounting for 12.69%.In addition,the attitude of informing accounts for 3.73%,(be)the object of informing accounts for 0.75%,the way of informing accounts for 2.24%,the disagree of informing(refused treatment)accounts for 5.22%,the proportion is relatively small.For the content of informing,the breakdown of the proportion as following: the incompleted informing accounts for 58.29%,the exaggerated and improper informing accounts for 17.14%,the concealed the truth accounts for 12.57%,the medication,equipment,charges are not informed accounts for 12%.Conclusions: 1.Medical humanities’ silence in the clinical field is the source that the medical staff can not fulfill the obligation to inform well.2.To master effective communication skills is the key to reduce medical complaints.3.That using appropriate and rigorous ways and more direct image means to inform can make the process of informing simple and the effect of informing be better.4.If the medical staffs can accurately grasp the time to inform and pay more attention to improve and retain the relevant informed consent documents,they can prevent the medical complaints effctively.5.Doctors should be suggesters in medical behavior.They also should give the patients space to make their own decisions and strictly protect the patients’ privacy.6.It is an effective means to regulate the format of the informed documents and signing processand to strengthen the hospital’s management and reduce medical complaints.7.As the third-grade class-A hospitals,it’s easier to cause complaints by informed consent in the provincial hospitals than in the municipal hospitals. |
