Objective and methods:The 1st hospital of Jilin Univercity have operated 99 distal pancreatectomy from January 2015 to December 2016,including 38 preserving spleen operations.Spleen preserving distal pancreatectomy includes kimura(26)and warshaw(12).The objective is making a Clinical retrospective study of the surgical duration,mean volume of introperative bleeding,postoperative fasting time,postoperative hospital stay and complication.Results: Compared Kimura and warshaw,there is no difference in the surgical duration,mean volume of introperative bleeding,intraoperative blood transfusion,postoperative fasting time,postoperative hospital stay and complication with no statistical difference.The incidence of pancreatic fistula between Endo-GIA and Endo-GIA with suture is 69% and 27%,the difference was statistically significant.Comparing the surgical duration,intraoperative blood transfusion,with the incidence of pancreatic fistula between Endo-GIA and Endo-GIA with suture,the difference was statistically significant.Comparing the the surgical duration,intraoperative blood transfusion,with the incidence of pancreatic fistula,there is positive statistical difference,there is negative correlation between diameter of tumor and,mean volume of introperative bleeding.Conclusions: Kimua is as safe as warshaw.Endo-GIA with suture can reduce the incidence of pancreatic fistula,The longer we cost in the operation and the more intraoperative blood transfusion,the higher incidence of pancreatic fistula will be.
