Objective: The study aims to investigate the prediction of cerebral microbleeds(CMBs),which is detected by susceptibility weighted imaging(SWI)in patients with ischemic stroke for secondary prevention.Methods: Prospective analysis 94 cases from may 2015 to april 2016 in the xinjiang uygur autonomous region people’s hospital of neurology department.CMBs on SWI were documented,record the quantity and distribution of CMBs,and all the patients take single anti-platelet drugs(aspirin 100 mg or clopidogrel 75mg),patients were follow-up through outpatient service or telephone at 6 months intervals.Primary end points were subsequent ICH,recurrent cerebral infarct(CI)and else.Results: A total of 94 patients were recruited.CMBs were identified in 50(53.2 %)patients.Mean follow-up period was 6.23 ± 0.7 months.Risk of subsequent ICH increased significantly with quantity of CMBs: 0%(no CMBs),6.7 %(1 CMBs),9.1 %(2–4 CMBs)and 15.4 %(≥ 5 CMBs)(P=0.015),Rate of recurrent CI was 9.1 %,6.7 %,22.1 % and 7.7 % respectively(P = 0.327).Conclusion: As tendency that recurrent CI exceed risk of ICH,anti-platelet drugs are still necessary.Risk of ICH increased with quantity of CMBs.However,in patients with ≥ 5 CMBs,the high risk of ICH seem to outweigh the modest benefit of anti-platelet drugs,so as clinical doctors should take extra precautions to minimize risk of ICH. |
