Objective: To explore the diagnostic value of Carbohydrate antigen125,Human epididymis protein4 and ROMA index in the diagnosis of early ovarian serous cancer.Methods: A retrospective analysis of January 2013-2016 year in December in Xinjiang cancer hospital from 386 cases of ovarian diseases were diagnosed including early ovarian serous carcinoma group and 86 cases of I 40 cases,II 46 cases),advanced serous ovarian cancer group of 105 cases(92 cases of stage III,13 stage IV),ovarian serous cystadenoma group and 94 cases of ovarian endometriosis cyst group,101 cases of preoperative CA125 and serum levels of HE4 and ROMA index values,by comparing between the groups,and the positive rate of serum level of sensitivity and specificity index,study the single tumor marker detection and combined detection in early diagnosis of ovarian serous carcinoma in value.Results:(1)Serum levels of CA125,HE4 and ROMA index were significantly higher in the early ovarian cancer group than in ovarian endometriosis group and ovarian serous cystadenoma group(all P=0.000),and all were P<0.05.These findings may help to screen for early serous ovarian cancer.(2)The positive rate of CA125 in early ovarian cancer group(80.23%)was higher than that of ROMA index(77.91%)and HE4(63.95%).The positive rate of CA125 in early ovarian cancer group was higher than that in ovarian endometriosis cyst(2?=2.004,P=0.106),P > 0.05,suggesting that there is no difference in the positive rate of CA125.The positive rates of HE4 and ROMA were statistically significant,and in the early ovarian cancer group compared with the ovarian serous cystadenoma group and the ovarian endometriosis cyst group(2?HE4=57.295,80.900,P=0.000,0.000;2?ROMA=91.891,67.929,P=0.000,0.000)were P < 0.05.(3)The positive rate of HE4 in ovarian serous carcinoma group was statistically different,Which stage I compared with stage II,III and IV(2?=8.782,24.952,11.304,P=0.003,0.000,0.000)were P < 0.05.The positive rates of ROMA in ovarian serous carcinoma group were statistically different,when stage I was compared with stage III and IV,(2?=23.534,4.511,P=0.000,0.030)all P < 0.05.The positive rate of CA125 in the different stages ofovarian serous carcinoma group(2?=2.188,P=0.135),P > 0.05.There was no significant correlation between the positive rate of CA125 and the stage of ovarian cancer..(4)Single tumor marker test: The specificity(95.90%),the positive predictive value(87.30%)and the diagnostic accuracy(86.12%)of HE4 in diagnosing early ovarian cancer were higher than those of CA125(51.28%,42.07%,60.14%)and ROMA(75.90%,58.77%,76.51%).The negative predictive value(88.62%)of ROMA was higher than that of CA125(85.47%)and HE4(85.78%).The ROC-AUC of ROMA(0.938)is greater than HE4(0.908)and CA125(0.804).(5)Combined detection: compared all positive the three tumor markers with either one or two were in the diagnosis of early ovarian serous cancer,the sensitivity(53.49%vs9.30%vs11.63%),specificity(98.46%vs98.46%vs93.85%),the positive predictivevalue(93.88%vs25.00%vs33.33%),thenegativepredictivevalue(82.76%vs69.31%vs69.58%),and diagnostic accuracy(84.70%vs68.68%vs67.26%)were increased.Conclusion:(1)When CA125,HE4 and ROMA index are all positive,it is more helpful for screening and diagnosis of early serous ovarian cancer.(2)The value of HE4 and ROMA in the diagnosis of early ovarian serous cancer is better than CA125.(3)CA125 is valuable in the diagnosis of early serous ovarian cancer,and is helpful for the differential diagnosis of benign diseases.(4)the positive rate of HE4 in ovarian cancer increased with the increase of ovarian cancer stage.
