TEAS Analgesic Effect Evaluation And Its Influence On Postpartum Milk Secretion And Postpartum Stress After Cesarean Delivery

Objective To observe TEAS analgesic effect after cesarean delivery and its influence on postpartum milk secretion,postpartum stress hormones and postpartum passage of gas by anus and so on.Method Patients were divided into three groups including treatment group(comfortable nursing + TEAS group),control group(comfortable nursing + PCA group)and blank group(comfortable nursing + fake TEAS group)with random and double blind method.(1)One hour after operation is regarded as before analgesia.The VAS score is applied to evaluate postpartum pain(before analgesia,5 minutes after analgesia,15 minutes after analgesia,30 minutes after analgesia,60 minutes after analgesia,24 hours after analgesia,48 hours after analgesia.)(2)Passage of gas by anus time of these three groups is compared.(3)Start time of lactation and the start time of lactation in second phase of these three groups are compared.(4)The blood serum corticotropin,prostaglandin and prolactin value of patients in the 3 groups at preoperative 24 hours,postoperative 24 hours and postoperative 48hours are compared.Results(1)Compared with blank group,VAS score of treatment group 30 minutes after analgesia is decreased significantly(P<0.05);from the 30 minutes after analgesia,VAS score has no significant difference between that of treatment group and of control group(P>0.05).(2)Towards start time of lactation,treatment group needs the time of 20.80±7.45h,control group needs the time of 18.50±5.85h,both of them are significantly earlier(P<0.05)than 25.06±7.67h of the blank group.Regarding the start time of lactation in second phase,treatment group needs the time of 46.45±12.42h,which is significantly(P<0.05)earlier than 55.10±13.76h of the control group.The preoperative and postoperative PRL values of the three groups are compared,with no statistical difference(P>0.05).(3)Time of passage of gas by anus in treatment group is significantly shorter than that of control group and blank group.(4)At preoperative 24 hours,postoperative 24 hour and postoperative 48 hours,the ACTH value of the three groups has no difference.(5)At preoperative 24 ours,postoperative 24 hours and postoperative 48 hours,the prostaglandin of the three groups has no difference.But compared with that at postoperative 24 hours,prostaglandin at postoperative 48 hours has been significantly decreased(P<0.05).Conclusion(1)TEAS has effective analgesia to cesarean section,its long-term analgesic effect(30min,24h,48h after operation)is equivalent to PCA.(2)TEAS can promote early passage of gas by anus and lactation,as well as premature lactation Ⅱ,but does not promote the production of prolactin.(3)TEAS and PCA analgesia won’t change the systemic stress levels,but can reduce prostaglandin production.
