| Objective: To study the relationships among demographic characters,H.pylori and gastrointestinal hormones in different subtypes of functional dyspepsia according to Rome IV in 2016.Methods : Consecutive functional dyspepsia(FD)patients who fulfill Rome IV criteria were evaluated at Affiliated Hospital of Southwest Medical University.Questionnaires on demographic characters,eating habits,drug use,assessment of gastrointestinal symptoms with Nepean Dyspepsia Index(NDI)and Dyspepsia Symptom Severity Index(DSSI),and Hamilton Anxiety and Depression Scale(HAMA/HAMD)were provided.Blood samples were collected for the detection of H.pylori CagA IgG,5-HT,ghrelin,gastrin,dopamine and cortisol.H.pylori was also determined with 14 C Urea Breath Test.Results: A total of194 patients were classified into postprandial distress syndrome(PDS,n=63),epigastric pain syndrome(EPS,n=40),overlap(n=22)groups and unexplained dyspepsia(n=69).The prevalence of FD subtypes differed with BMI(p=0.034)education(p=0.004)and use of NSAIDS or antibiotics(p=0.016).The infection of H.pylori was different in FD subtypes(p =0.040).The sore of HAMA was significantly higher in Overlap than PDS or EPS(p=0.001).No significant differences can be found in FD subtypes,concerning the level of 5-HT,ghrelin,gastrin,dopamine and cortisol.But 5-HT,gastrin,ghrelin is significantly different between H.pylori positive patients and H.pylori negative.Conclusions:H.pylori infection and anxiety is associated with FD,and it could be possible cause of FD symptoms.H.pylori infection is related to 5-HT level in FD patients,and they may play a role in the occurrence and development of FD each other.Unexplained dyspepsia could be the early manifestations of FD,treatment or intervention to prevent the provenance of FD need to be considered.High BMI,antibiotics usage,anxiety or H.pylori infection could promote the presence of FD for patients with unexplained dyspepsia.H.pylori positive patients is more likely to occur PDS,eradication of H.pylori may improve the symptom remission rate. |
