Objective: To investigate the relationship between interleukin-1β-511(C/T)gene polymorphism and early diabetic kidney disease(EDKD)in Han population of Luzhou area,and to explore the risk factors of EDKD,which would be provide a theoretical basis for diagnosis,prevention and treatment of EDKD.Methods : Based on the epidemiology investigation of unrelated Han population aged 40 years and above from April to November in 2011,548 patients with type 2 diabetes(T2DM)were selected and divided into two groups: normal albuminuria group(NA group,n=286)and micro-albuminuria group(MA group,n=262).327 healthy subjects were enrolled as normal controls(NC group,n=327).The polymorphisms of IL-1 gene were analyzed by the polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP).At the same time,the values of height,weight,waist circumference,hip circumference,systolic pressure blood(SBP),diastolic blood pressure(DBP),Glycosylated hemoglobin(HbA1c),fasting plasma glucose(FPG),2-hour postprandial blood glucose(2hPG),triglyceride(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),lowdensity lipoprotein cholesterol(LDL-C),serum creatinine(Scr),urine creatinine(Ucr),micro-albuminuria(mAlb)were measured and body mass index(BMI),Waist-to-Hip Ratio(WHR),glomerular filtration rate(eGFR),urinealbumin-creatinine ratio(UACR)were counted.Data interpretation and analysis was carried out using SPSS 17.0.Results: 1.Our results showed a significant differences in the genotypes and alleles of the-5ll region of IL-1βamong the three groups(P<0.05).Compared with the control group and NA group,the distribution of the-5ll region of IL-1β genotypic frequency was significantly different(?2=6.651,P=0.036;?2=9.004,P=0.011),and the distribution of allele frequency also different(?2=5.204,P=0.023;?2=8.311,P=0.004).2.There weren’t significant different of genotypes frequency [CC、CT、TT=24.6%,48.0%,27.4%(T2DM group)vs.23.3%、55.5%、21.7%(NC group)?2=4.775,P>0.05] and allele frequency [C、T=48.6%、51.4%(T2DM group)vs.50.8%、49.2%(NC group)?2=0.746,P>0.05)] of IL-1β-511 between T2 DM group and NC group.There weren’t statically significant different in the genotypes(?2=0.746,P>0.05)and allele frequency(?2=5.197,P=0.074)of the-5ll region of IL-1β between NA group and NC group.3.Comparison of baseline data and biochemical index among different IL-1β-511 genotypes: the level of HbA1 c were significant difference among the three genotypes groups(P<0.05),TT genotype were higher than CT genotype(P<0.05)and CC genotype(P>0.05);and the level of UACR were significant difference among the three genotypes groups(P<0.05),CC genotype were lower than TT genotype and CT genotype(all P<0.05).4.Comparison of base data and biochemical index among three groups.Compared with NC group,the age,SBP,DBP,FPG,2hPG,HbA1 c,TG,TC,LDL-C,BMI,WHR,eGFR,UACR were significantly higher in MA group(P<0.01).Compared with NA group,the age,SBP,DBP,FPG,2hPG,TG,TC,LDL-C,eGFR,UACR were higher in MA group(P<0.01).5.Logistic regression analysis indicated that the 2hPG,TC,SBP,TT+CT genotype were the independent risk factors for EDKD.Conclusion: 1.the interleukin-1β gene-511C/T polymorphism exists in Luzhou Han population.2.There weren’t significant different of genotypes frequency and allele frequency of IL-1β-511 between T2 DM group and NC group.There weren’t enough association of the polymorphism with T2 DM.3.The IL-1β-511C/T gene polymorphism may association with the development of EDKD in Han nationality of Luzhou area.The T allele may through affect the levels of HbA1 c,UACR to increase the risk of EDKD.4.2hPG,TC,SBP were the independent risk factors for early diabetic kidney disease.Enhance relevant knowledge publicity to the community residents,strengthen detection and control blood glucose,blood pressure,and blood lipids are important to prevention and treatment early diabetic kidney disease.
