Objective: To observe the effect of Danzhi Jiangtang Capsule on serum atrial natriuretic peptide,angiopoietin like protein 2 and bradykinin β2 receptor in patients with type 2 diabetes mellitus with hypertension;to investigate the correlation between type 2 diabetes mellitus and hypertension and inflammatory factors,the mechanism of inflammatory factors leading to the disease,to evaluate the effect of DJC on the inhibition of inflammatory response,and to explore the target and mechanism of DJC on the disease and its complications.Method: 1.Clinical Research: 1.1 Research Objects: All 84 patients were selected from the First Affiliated Hospital of Anhui University of traditional Chinese medicine from May 2015 to August 2016.The patients with type 2 diabetes mellitus with hypertension were divided into treatment group(n=42)and normal control group(n=42).1.2 Research Methods: Two groups of patients were selected to carry out life intervention.Control group with hypoglycemic medicine,treatment group with western medicine and DJC.The two groups were treated for 12 weeks.TCM syndrome score,systolic blood pressure,diastolic blood pressure,echocardiography correlation values of the two groups of patients were recorded before and after treatment,blood glucose(FPG,2h PG),glycosylated hemoglobin(Hb A1c),blood lipids(TC,TG,LDL,HDL),high sensitive C reactive protein(hs-CRP),ANP,Angptl2.The differences between the two groups before and after treatment were compared.1.3 clinical Findings Clinical study found,after treatment,the total effective rate of Control group was 58.29%,the total effective rate of Treatment group was 92.50%.the treatment group was better than the control group(P<0.05).control group and treatment group were significantly lower than before treatment,the difference was very significant(P<0.01).After 12 weeks of treatment,compared with the control group,of treatment group decreased more significantly,with statistical significance.Compared with before treatment,the score,FPG,2h FPG,Hb A1 c,SBP,DBP,TC,TG and LDL in two groups were significantly decreased(P<0.01),HDL increased significantly(P<0.01).The antihypertensive and Hypoglycemic effect of the treatment group was better than that of the western medicine group.LVEF and E/A ratio of the treatment control group compared with those before treatment were significantly different(P<0.05).after treatment,two groups patients’ ANP,Angptl2 and hs-CRP levels were lower than before treatment,the difference is very significant(P<0.01).after treatment,there was a significant difference between two groups patients’ the levels of ANP,Angptl2,hs-CRP(P<0.05 or P<0.01).2.Animal Experiment 2.1 Research Subjects: 59 male SD rats,fed with basic feed for 7 days,9 rats were randomly chosen as normal control group,to continue feeding with basal diet;the other rats were used as experimental group,fed with high fat feed premix.after 4 weeks,the experimental group rats by intraperitoneal injection of STZ(35mg/kg).After the success of the model,will become the model of diabetic rats were randomly divided into 4 groups: model group,high DJC dose group,low DJC dose group,pioglitazone group.Intragastric administration every day,for 8 weeks.2.2 Methods : The experimental rats were fasted for 12 hours,with 3% chloral hydrate intraperitoneal injection of 10mg/kg rapid extraction anesthesia,abdominal aortic blood,a part of TC for the detection of FPG and TG,another part of cryopreservation for serum B2 R and Angptl2.2.3 Experiment Results compared with the normal control group,model group in different periods of blood glucose were significantly increased(P<0.01);Compared with the model group,DJC high dose group,DJC low dose group,pioglitazone group blood glucose levels on the 4 weekend and 8 weekend were significantly decreased than the model group(P<0.01 or P<0.05).Compared with normal control group,the levels of TG and TC in rats of model group were significantly higher(P<0.01).After treatment,the levels of TG and TC in high DJC dose group,DJC low dose group and pioglitazone group were significantly lower than those in model group(P<0.01).Compared with the normal control group,the serum B2 R and Angptl2 level in model group increased significantly(P<0.01);after intragastric administration,the levels of B2 R and Angptl2 in DJC high dose group,DJC dow dose group and pioglitazone group were significantly decreased(P<0.01).Pearson correlation analysis showed that B2 R has a highly linear positive correlation with Hb A1 c,the correlation coefficient(r=0.744),Angptl2 and Hb A1 c have a significant linear positive correlation,the correlation coefficient(r=0.695).Conclusion: Danzhi Jiangtang Capsule could increase in diabetes patients with hypertension of TCM clinical symptoms improved,can reduce blood lipids and high expression of B2 R,ANP,hs-CRP,Angptl2,inflammatory factors and other related indicators,it may play a role through the correct glucose and lipid metabolism and improve insulin function.
