Study On The Relationship Between The TCM Pathological Features And Insulin Resistance,Its Related Clinical Indicates In Type 2 Diabetes Mellitus

Objective The thesis aim to investigate the relationship between TCM Pathological features and Insulin Resistance,its related clinical indicates in patients with T2DM.On the one hand,it is hoped that it will provide clinical evidence for preventing the development of T2DM,the progress of chronic diabetic complications,and personal treatment programs.Methods Collected data from January 2016 to December 2016 in-patient treatment in the People's Hospital of Fujian University of TCM Department of Endocrinology and cardiovascular,224 cases of T2DM,according to Professor Wang Qi's body quality question-naire for the patients to determine the TCM constitution,and use of the four diagnostic information to identific the TCM Pathological.At the same time to collect the general information of the patients such as sex,age,date of birth,height,weight,and blood was collected for determination of fasting insulin,fasting blood glucose,total cholesterol,trigly-ceride,high density lipoprotein,low density lipoprotein,thrombin time,prothrombin time,activated partial thromboplastin time,D-dimer,fibrinogen.Collected data were statistica-lly analyzed by SPSS 20.0.Results1.? Distribution regular of TCM constitution:The TCM constitution with T2DM is phlegm-dampness,yin-deficiency,bloodstasis,qi-deficiency,qi-stagnation,damp-heat,yang-defi ciency,peaceful quality,special intrinsic quality,the proportion was 27.7%,25.4%,3.4%,11.6%,8.50%,4.9%,4%,3.6%and 0.9%,respectively.?Distribution regular of TCM syndrome:The main locations of the disease are spleen,kidney,liver,lung and mind;deficiency of Yin,phlegm,damp,deficiency of Blood,deficiency of Qi are the main characteristic of the nature of T2DM.2.There were differences in sex distribution(P<0.05),but no significant difference in the distribution of different age groups and different course among TCM constitution in patients with T2DM.3.There was a significant difference in BMI between different TCM constitution in patients with T2DM(P<0.05).Phlegm-dampness was positively correlated with BMI,the correlation coefficient was 0.352.4.?Statistical analysis of TCM constitution and Insulin Resistance the results showed that:there were significant differences in Insulin Resistance index distribution between different TCM constitution in patients with T2DM(P<0.05),the difference was statistically significant(P<0.01),phlegm-dampness was positively correlated with Insulin Resistance(r=0.438,P<0.01).?Statistical analysis of TCM syndrome and Insulin Resistance the results showed that:spleen was positively correlated with Insulin Resistance(r?0.307,P<0.01).5.?Statistical analysis of TCM constitution and chemical indicators,the results showed that:there were differences between TC and LDL between different TCM constitution in patients with T2DM(P<0.05),phlegm-dampness was positively correlated with TC and LDL(P<0.001),the correlation coefficients were 0.444 and 0.394 respectively.There were signi-ficant differences in FIB between different TCM constitution in patients with T2DM(P<0.05),bloodstasis and FIB were positively correlated with a correlation coefficient of 0.239.?Statistical analysis of TCM syndrome and chemical indicators,the results showed that:spleen was positively correlated with TG(P<0.05,r=0.136).Conclusion1.Phlegm-dampness,yin-deficiency,bloodstasis,qi-deficiency and qi-stagnation are the common TCM consititution in T2DM.2.The disease course getting longer,extended refractory,the main locations of the disease is spleen,it can be involved kidney,liver,lung and mind,it appearing inclusion of deficiency and excess.Phlegm dampness is the pathogenic factors of this disease,but also the pathologi-cal products of the disease,deficiency of Yin is an important pathological basis of this disease.In the course of treatment should pay attention to the conditioning of the spleen and kidney,while taking into account the liver,the proposed patients to maintain emotional ease.3.Deficiency of Spleen and phlegm pampness are the important TCM pathological features of T2DM,with the increase of BMI,the degree of insulin resistance increased,reduce the BMI and conditioning the function of spleen can regulate insulin resistance.The patients with total cholesterol,low density lipoprotein disorders,so the clinical treatment of the spleen can be regulate phlegm-dampness constitution,thereby improving blood lipid disorders.Blood stasis in patients with high FIB,blood hypercoagulable state easy to form thrombosis,leading to the occurrence of diabetes-associatedchronic complications,clinical attention to the blood stasis constitution to relieve blood stasis in patients with state.
