| ObjectiveComparing brackets and plasters immobilization in the treatment of acute ankle sprain,and exploring the clinical efficacy of new brace on acute ankle sprain based on the POLICE principle.MethodsA prospective controlled study was performed.Patients with acute ankle sprain were within 24 hours were collected.Patients were divided into three groups:test group(A),control group(B)and blank group(C).And every group accepted the listing treatment.Test group(A):after treatment,bandage after the new brace to fix ankle joint 2 to 3 weeks.Ice within 24h after injure(each 15~20min,once every 2 hours,continuous 4~72 hour),rest within 3 days after injury,raise the limb,active toes to relieve swelling,patients can walk properly indoor after 3 days in the case of pain can be tolerated,and the patients can use axis support if can not walk because of the unbearable pain.Control group(B):bandage after dressing blaster fixed ankle joint 2 to 3 weeks.Ice within 24h after injure(each 15~20min,once every 2 hours,continuous 48 to 72 hours),raise the limb within 3 days after injury,active toes to relieve swelling,avoid under-weight in the blster-fixed period.The functional exercise during the fixed period is the same as brace group.Removal of blaster after 2 or 3 weeks,they can walk indoor after 3 days in the case of pain can be tolerated,and the patients can use single-axis support if can not walk because of the unbearable pain.Blank group(C):without brace and blaster,just use bandage after injury,ice within 24h after injure(each 15~20min,once every 2 hours,continuous 48 to 72 hours),rest within 3 days after injury,raise the limb,active toes to relieve swelling,patients can walk properly indoor after 3 days in the case of pain can be tolerated,and the patients can use axis support if can not walk because of the unbearable pain.The scores of Povacz lateral collateral ligament,Kofoed score,VAS score and clinical symptom score were collected in each group after 3days,1 week,2 weeks and 4 weeks after treatment.After 4 weeks of treatment,the improvement rate,recovery time and satisfaction were recorded,and the clinical curative effect was evaluated comprehensively.After treating 12weeks,calculate the number of cases of sprains again and and the complication incidence in each group.Results1.The Povacz lateral collateral ligament score and Kofoed score were significantly higher than that of the plaster group and the blank group at 1 week and 2 weeks after treatment(P<0.01),and the clinical symptoms and signs were significantly lower than the control group and the blank group(P<0.01).2.There were no significant difference in the scores of Povacz lateral collateral ligament,Kofoed score,and clinical symptoms,signs,VAS score and improvement rate amony 3 roupsafter 4 weeks of treatment(P>0.05).However,the satisfaction rate of brace group were significantly higher than that of blaster group and blank group(P<0.01),and the recovery time was significantly shorter than that of blaster group(P<0.01).And the complication rate of brace group were lower than that of the other two groups(P<0.01).3.The follow-up indexes of bracelet group and gypsum group were better than those of blank group(P<0.05).The complication rate of blank group were generally higher than that of brace group and blaster group(P<0.01).Conclusion1.Treating acute ankle sprain by using new brace based on the principle of POLICE,can ease the pain,swelling,improve ankle function more quickly than the plaster fixation.2.Fixation treatment is very important in the treatment of acute ankle sprains.The short-term clinical effect of the new brace is similar to that of the plaster.However,new type of brace fixation has higher therapeutic satisfaction than plaster fixation,and it can shorten the course of disease and reduce the complication rate.The new type of brace is worthy of clinical application. |
