Objective:To retrospectively analyze the clinical features of patients with Hodgkin lymphoma(HL),evaluated the therapeutic effect,and explore the prognostic factors.Method:The clinical data of 154 primary HL patients in the First Affiliated Hospital of Zhejiang University School of Medicine collected from January 2010 to October 2016 were analyzed retrospectively.The clinical features of the patients were analyzed retrospectively,and the therapeutic effect was evaluated.The prognostic factors were calculated using the Kaplan-Meier method in univariate analysis and COX proportional Hazard Model in multivariate survival analysis.Results:(1)A total of 154 cases of HL were included with a median age of 36.5 years(range 14-80 years).59.7 percent of patients were male and 40.3%were female.The overall survival was 94.2%.All patients received ABVD or AVD chemotherapy for median 6 cycles,and 91.6%achieved a complete remission or partial remission.20.1%of the patients went through relapse and 5.8%died.The median PFS was 22.5 months for 154 HL patients and 12 months for 47 patients with recurrent or disease progression.(2)HL patient with factor as Male(P=0.016),B symptoms(P<0.001),Ann Arbor stage ?-?(P=0.04),IPI?2(P=0.032)and CRP>25mg/L(P=0.004)has lower 3 year-PFS rate with significance.One year-PFS rate was significant difference between patients with or without B symptoms.(3)Univariate analysis suggested that male(P=0.004),B symptoms(P<0.001),Ann Arbor stage ?-?(P=0.006),IPI?2(P=0.001),IPS>2(P=0.006),ALC<0.6×109/L(P=0.004),CRP>25mg/L(P=0.001)and LDH>280?mol/L(P=0.013)significantly reduce PFS in HL patients.(4)Multivariate analysis revealed that factor male(HR 2.309,95%CI 1.131-4.714,P=0.022)and B symptoms(HR 2.659,95%CI 1.280-5.524,P=0.009)shorten PFS with significance independently.(5)There were 54 patients(35.1%)with stage ?-? and 100 patients(64.9%)with stage ?-?.The overall survival rates were 98.1%and 92%,respectively without significant difference(P=0.121).The 3-year PFS rate was significantly higlher in stage?-? patients than in stage ?-?(83.3%vs 68%,P=0.04).Male,B symptoms and ALC<0.6×109/L are negative prognosis factors of PFS for patients with ?-? stage HL.Conclusions:Hodgkin Lymphoma patients have favorable outcome.Male and B symptoms were independently correlated with shorter PFS of HL patients.Male,B symptoms and ALC<0.6×109/L have predictive value for PFS in HL patients with ?-? stage HL.
