Background:Irritable bowel syndrome(IBS)is one of the most commom gastro-intestinal disorders, characterized by abdominal pain or abdominal discomfort, with changes of defecation frequency and/or stool characteristics.Over the past few decades, some diagnostic criterias have been developed based on symptoms of IBS.In 2016,the latest diagnostic criteria for IBS-Rome Ⅳ criteria was released.IBS is a functional gastrointestinal diseasecaused by multiple factors, the risk factors are complex and diverse. The diet, climate, environmental and other factors are significantly different between Wuhan and Dalian.so,the incidence factors, morbidity, severity of clinical symptoms and subtype classification ratio may be different. There is still lack of literatures on the study of IBS using the Rome Ⅳ criteria,so are the literatures researing the differences of IBS clinical characteristics from different areas in China using Rome Ⅰ、Ⅱ、Ⅲ criteria. So,in this paper,we were trying to make a simple comparison of the general situation, morbidity, subtype, symptom severity and quality of life between Wuhan and Dalian based on the Rome Ⅳ criteria, and explore the possible causes of these differences in order to provide reference for the follow-up treatment.Purpose:We compared the differences in the general condition, symptom severity, quality of life, anxiety and depression between the positive and negative patients(based on the Rome Ⅳcriteria)with suspectedIBS in Wuhan and Dalian by questionnaires. And we compared the differences of the positive rate of Rome Ⅳ,the subtype, the severity of symptoms, the quality of IBS,the quality of life, the anxiety and depression, and the related factors between the severity and quality of life of positive patients(based on the Rome Ⅳcriteria)with suspected IBSin Wuhan and Dalian. So that we could find the differences of clinical of IBS characteristics between the two areas. Methods:A total of 132 patients with irritable bowel syndrome in Wuhan were collected, and 122 patients were collected from Dalian. They all measured up the inclusion and exclusion criteria. The IBS patients of Wuhan area who met Rome IV were assigned as the W(IBS)group and the rest were assigned as the W(N-IBS)group.Alao,The IBS patients of Dalian area were divided into D(IBS)and D(N-IBS)group.Patients were asked to complete the questionnaires, including the general conditions of the patients, the IBS-related symptoms, the Irritable Bowel Severity Scoring System(IBS-SSS),the Gastrointestinal Sypmtom Rating Scale(GSRS),Quality of Life in Persons with Irritable Bowel Syndrome(IBS-QOL)and the Hospital Anxiety and Depression Scale(HADS).The statistical methods ofchi-square test,Mea±standard deviation,t-test and multiple linear regression analysiswere used to analysis(SPSS17.0).Result:1、There was no significant difference between W(IBS)and W(N-IBS)in generally conditions(P>0.05),the same as D(IBS)and D(N-IBS)group.The scores of GSRS,IBS-SSS and IBS-QOL had significantly different between W(IBS)and W(N-IBS)(P<0.05).And the symptoms severity of W(IBS)and D(IBS)was worse, and the quality of life was lower than that of W(N-IBS)and D(N-IBS).(P<0.05).The ratio of W(IBS)and D(IBS)group was also higher than that of W(N-IBS)and D(N-IBS)group on the severity of IBS-SSS severity. There was no significant difference in the scores of anxiety, depression and severity between W(IBS),D(IBS)group and W(N-IBS),D(N-IBS)group(P>0.05).2、There was no statistically significant difference between W(IBS)and D(IBS)in general condition(P>0.05)except IBM.And there was also no significant difference in Rome IV positive rate between the two groups(P>0.05).3、In the comparison of IBS subtype,The D(IBS)group had more IBS-D patients comparied with W(IBS)group(P<0.05).However,There were no significant difference between the other subtypes(P>0.05).4、There was no significant difference in the score of GSRS between W(IBS)and D(IBS)group(P>0.05)as a whole. But there were significant difference in Indigestion and intestinal dysfunctionsymptoms groups between W(IBS)and D(IBS)groups(P<0.05).5、There was significant difference in the score of IBS-SSS between W(IBS)and D(IBS)group(P>0.05)as a whole.Compared with W(IBS)group,D(IBS)group was more serious in IBS-SSS.But there were no significant differences in the grade of IBS-SSS between the two groups(P>0.05).6、There was no significant difference in the score of IBS-QOL between the W(IBS)and D(IBS)groups(P>0.05)as a whole. However,There was significant difference between in disorder, somatic idea, food escape and sexual behavior between the two groups(P>0.05).7、There was no significant difference in the scores of anxiety and depression between theW(IBS)and D(IBS)groups(P>0.05).8、In the multiple linear regression analysis, the anxiety of W(IBS)and D(IBS)had an effect on the severity of GSRS,IBS-SSS and IBS-QOL(P<0.05).Conclusion:1、According to the Rome IV criteria, the positive rate of IBS patients betweenWuhan and Dalian had no significant difference. There was no difference between the two areas in general conditions(eg age,sex,etc.).The IBS patients were severer in symptoms and more worse in quality of life in Dalian than that of in Wuhan. There was no difference in anxiety and depression between the two areas, but anxiety had an effect on the severity of symptoms and quality of life in IBS patients.2、In the IBS subtype, the ratio of IBS-D in Dalian was higher than Wuhan. However, the other three subtypes had no significant difference.3、There were no significant differences in the general conditions between the IBS patients met Rome IV criteria and the rest patients not met Rome IV criteria in Wuhan area, but the former was more serious in symptoms and was more worse in the quality of life. The patients in Dalian also had the same results.
