Objective:The aim of this research is to improve the quality of the recovery of the patients after VATS,with the thought of ERAS which had been hot spot of clinical recent years,to compare the differerce of recovery between perioperative intravenous lidocaine versus dexmedetomidine infusion.Methods:This double-blind,radomized trail was approved by the Institutional Review Board at the second affiliated hospital of Dalian medical university.Written Informed consent was obtained from all paticipants.According to the inclusion criteria,we enrolled 100 patients aged 50-75 yrs old with an ASA physical status ?-? who were scheduled for elective VATS for lung cancer from July 2016 to December 2016.Each patient was randomized to Lidocaine group,dexmedetomidine group or control group.The medical record data and the detailinformation of surgery and anesthesia,also the information of therecovery postopetation were collected,including age,sex,height,weight,BMI,history of smoking and drinking,ASA physical status,duration of operation and anesthesia,time of one-lung ventilation,dose of lidocaine and dexmedetomidine,time of PACU,the concentration of TNF-? at inducing and ending time of anesthesia,QoR-40 score,the return of bowel function,the VAS score at 24 h,48h,72 h postoperative.Then we conducted a statistic analysis on all data we collected from every patient and get the result.Results:(1)The postoperative hospital stay of group lidocaine2.0 was significantly shorter than the other four groups(P<0.01).(2)The fart time of group lidocaine2.0 and group dexmedetomidine0.7 were significantly shorter than the other four groups(P<0.01).(3)The QoR-40 score of group lidocaine1.5 and group lidocaine2.0 were superior to the other groups(P<0.05).(4)The VAS score of group lidocaine1.5 and group lidocaine2.0 were lower to the other groups(P<0.05).(5)The degree of change before and after surgery of TNF-? in plasma of group lidocaine2.0 and group dexmedetomidine0.7 were significant than the other groups(P<0.05).The degree of change before and after surgery of TNF-? in plasma were significant in group lidocaine2.0,group dexmedetomidine0.4,group dexmedetomidine0.7and control group(P<0.05).Conclusions: Intravenous lidocaine improves the quality of recovery after Single-hole VATS,and has the effect of anti-inflammatory,analgesic,shortening the postoperative hospital stay,promoting the recovery of gastrointestinal function,and is helpful to strategy of ERAS.
