| Objective:To explore the value of contrast enchanced ultrasound(CEUS)with enhanced magnetic resonance imaging(enhanced MRI)for prostate cancer detection in different range of PSA.Methods: 135 patients suspected prostate cancer patients were retrospectively analyzed and all the patients underwent CEUS and enhanced MRI.And all patients further performed prostate biopsy through transrectal for prostate cancer final diagnosis.The result pathological of biopsy were performed to evaluate the diagnostic value of CEUS and enhanced MRI.Result:In diagnostic prostate disease,the sensitivity of CEUS,enhanced MRI and The combination of the two were 75.43%、80.70%、92.98%respectively,the specificity were 77.33%、86.67%、73.33% respectively.The sensitivity of combination is higher than any separate diagnostic technology.There are significant difference compare the combination of two with separate diagnostic technology in the sensitivity(P<0.05).The specificity of enhanced MRI is higher than the combination or CEUS.There are significant difference compare the enhanced MRI with the combination or CEUS diagnostic technology in the specificity(P<0.05).When PSA in the range of 4.0ng/mL to 10.0 ng/mL,The specificity of CEUS,MRI and the combined application were82%,96%,80% respectively.There were significant differences compare MRI enhancement and combined diagnosis in diagnosis of prostate cancer(P<0.05).When PSA in the range of 10.0ng/mL to 20.0ng/mL The sensitivity of CEUS,MRI and the combined application to the diagnosis of prostate cancer were 42.86%,52.38% and 80.95%respectively,and the specificity was 73.68%,68.42% and 63.16% respectively.When PSA more than 20.0ng/mL,the cases of prostate cancer were all diagnosed correctly with three kinds of diagnostic technology.Conclusion: The sensitivity of the combined diagnosis is better than CEUS or MRI for the diagnosis of prostate cancer.The specificity of enhancedMRI for the diagnosis of prostate cancer is better than the combined diagnostic technology.When PSA in the range of 4.0 ng/mL to 10.0 ng/mL,MRI enhanced diagnostic technology has high specificity.So the MRI enhanced has diagnostic advantage,when PSA in the range of 10.0 ng/mL to 20.0 ng/mL,the combined diagnostic technology has high sensitivity.The combined application has the advantage of diagnosis.When PSA more than 20.0 ng/mL,simple CEUS check can improve the positioning accuracy of prostate biopsy.So CEUS diagnosis has advantages. |
