Objective: To investigate the changes of left heart structure and function in patients with paroxysmal atrial fibrillation after circumferential pulmonary vein isolation catheter ablation in three months.Methods: Patients with paroxysmal atrial fibrillation, which underwent circumferential pulmonary vein isolation catheter ablation, were collected in the first affiliated hospital of Guangxi medical university from October 2014 to October 2015. The chest doppler echocardiography was performed under sinus rhythm to measure inner diameter and left atrial diameter of the left atrium, left atrial length to diameter, left ventricular end-diastolic diameter, left ventricular end systolic diameter, left ventricular ejection fraction in the preoperative 24-48 hours and postoperative 3 months, respectively. And compared the changes of the left heart structure and function between the preoperative 24-48 hours and postoperative 3 month. And comparation of the changes in left heart structure and function were performed between the preoperative 24-48 hours and postoperative 3 months.Results: All the patients with atrial fibr:illation (af) were successfully finished the operation. Of which 44 cases were sinus rhythm after following up for 3 months postoperatively. The postoperative follow-up process, the patients who successfully maintained sinus rhythm: 1) The left atrial diameter preoperative VS postoperative 3 months respectively was (38.00 + 3.61) mm VS(35.40 + 5.27) mm, left atrium diameter preoperative VS postoperative 3 months respectively was (42.20 + 3.63) mm VS (41.40 + 4.39) mm, left atrium diameter preoperative VS postoperative 3 months respectively was (59.80 + 8.26) mm VS(55.60 + 6.69) mm. There was statistically significant difference between the two periods (P < 0.05). The results indicated that the patients 3 months after circumferential pulmonary vein isolation and maintained sinus rhythm successfully, the left atrium diameter, left atrium diameter andlong lefit atrium diameter were significantly decreased when compared with the preoperative parameters; 2 ) Left ventricular end-diastolic diameter preoperative VS postoperative 3 months respectively was (50.66 + 4.2 VS 4) mm VS (50.50 +4.10) mm, left ventricular end systolic diameter VS postoperative 3 months respectively was (32.39 + 4.38) mm VS (31.84 + 3.44) mm, left ventricular ejection fraction preoperatively VS postoperative 3 months respectively was(60.93±5.57 % VS (61.77±6.46) %, p values were greater than 0.05, there was no statistically significant difference. Results indicated that the patients 3 months after circumferentialpulmonary vein isolation and maintained sinus rhythm successfully, left ventricular end-diastolic diameter andleft ventricular end systolic diameter were less, while left ventricular ejection fraction was improved compared with the preoperative, but there was no statistically significant difference.Conclusion: 3 months after the circumferential pulmonary vein isolation,if can maintain sinus rhythm, patients with the left atrial structure was obviously narrowed, left ventricular structure and function were not obviously effected.
