Objective: Chronic obstructive pulmonary disease(COPD)is a common disease and frequently encountered disease of respiratory system,its incidence is increasing every year.The pathogenesis of COPD is not entirely clear at present.Inflammation mechanism is widely regarded as one of the pathogenesis now.Finding inflammation-related genes and their expression factor is very important to study the pathogenesis of COPD.This research through studying TREM-1 mRNA expression in the peripheral blood mononuclear cell nucleus and the concentration of serum sTREM-1,IL-1β in different patients with COPD,to explore correlation between TREM-1 mRNA expression and the concentration of serum sTREM-1 and IL-1β,and the role of TREM-1 in the pathogenesis of COPD and the relationship between airway inflammation.Methods: Sixty acute exacerbation of COPD patients were collected from September 2015 to March 2016 in Affiliated Hospital of North Sichuan Medical College as AECOPD group.and after active treatment,such as anti-infection,smooth wheezing and oxygen inhalation,the patients condition get better and reach the stable state as SCOPD group.According to whether patients with the combination of respiratory failure,AECOPD group patients were divided into AECOPD with respiratory failure and AECOPD without respiratory failure.According to pulmonary function testing(FEV1/FVC,FEV1 predicted%),a comprehensive assessment of exacerbations in the past year and whether hospitalized when COPD exacerbations,patients were divided into low risk group and high risk group,30 people in each group.30 healthy examiners[mean age(66.45±9.92)years] were chosen as healthy control group in the outpatient and Physical examination center on the corresponding period.Enzyme linked immunosorbent assay(ELISA)was performed to measure the serum levels of soluble triggering receptor exprssed on myeloid cells-1(sTREM-1)and interleukin(IL)-1β,and quantitative real-time PCR was used to detect the mRNA of TREM-1 in PBMCs in the COPD group and healthy group.Analyzing the change of TREM-1 mRNA expression of patients with COPD and exploring correlation between TREM-1 mRNA expression and the concentration of serum sTREM-1,IL-1β in different groups,explore severity relationship between COPD patients with TREM-1 mRNA expression and concentration of sTREM-1,IL-1β in different grouping and their correlation,and the correlation between TREM-1 mRNA expression and concentration of sTREM-1,IL-1β with FEV1%pred.Result:(1)The expression of TREM-1mRNA in peripheral blood mononuclear cell nucleus in COPD patients group with acute exacerbation,COPD stable group and healthy control group were 9.69±2.89,3.33±1.87 and 1.04±0.94.Acute exacerbations of COPD group and COPD stable group compared with healthy control group,TREM-1mRNA expression of COPD patients with acute exacerbation COPD group and COPD stable group were significantly higher than healthy control group,the difference was statistically significant(P<0.01).Comparing healthy control group,TREM-1mRNA expression of COPD stable group was significantly higher,the difference was statistically significant(P<0.01);The sTREM-1 concentration in serum of AECOPD group,SCOPD group and healthy control group were 106.31±10.52pg/ml,81.13±7.67pg/ml,63.04±8.02pg/ml.The concentration of serum sTREM-1 in AECOPD group and SCOPD group were significantly higher than healthy control group,the difference was statistically significant(P<0.01).Comparing healthy control group,the sTREM-1 concentration in serum of SCOPD group was significantly higher,the difference was statistically significant(P<0.01);The IL-1β concentration in serum of AECOPD group,SCOPD group and healthy control group were 68.71±9.60pg/ml,40.40±3.66pg/ml,23.55±6.93pg/ml.The concentration of serum IL-1β in AECOPD group and SCOPD group were significantly higher than healthy control group,the difference was statistically significant(P<0.01).Comparing healthy control group,the IL-1β concentration in serum of SCOPD group was significantly higher,the difference was statistically significant(P<0.01).(2)The expression of TREM-1mRNA in peripheral blood mononuclear cell nucleus in AECOPD with respiratory failure group and AECOPD without respiratory failure group were 12.12±3.61 and 9.14±3.12.The expression of TREM-1mRNA of AECOPD with respiratory failure group higher than AECOPD without respiratory failure group,the difference was significant(P<0.05);The sTREM-1 concentration in serum of AECOPD with respiratory failure group and AECOPD without respiratory failure group were 122.71±8.51pg/ml and 104.22±10.18pg/ml.The sTREM-1 concentration in serum of AECOPD with respiratory failure group higher than AECOPD without respiratory failure group,the difference was significant(P<0.05);The IL-1β concentration in serum of AECOPD with respiratory failure group and AECOPD without respiratory failure group were 81.13±10.42pg/ml and 67.94±9.74pg/ml.The concentration of serum IL-1β in AECOPD with respiratory failure group higher than AECOPD without respiratory failure group,the difference was significant(P<0.05).(3)The expression of TREM-1mRNA in peripheral blood mononuclear cell nucleus in SCOPD with high-risk group and SCOPD with low-risk group were 4.73±1.33 and 1.94±1.14.SCOPD with high-risk group compared with SCOPD with low-risk group,the TREM-1mRNA expression of SCOPD patients with high-risk group were higher than SCOPD with low-risk group,the difference was statistically significant(P<0.05);The sTREM-1 concentration in serum of SCOPD patients with high-risk group and SCOPD with low-risk group were 87.70±5.57pg/ml and 75.20±3.97pg/ml.The concentration of serum sTREM-1 in SCOPD patients with high-risk group were higher than SCOPD with low-risk group,the difference was significant(P<0.05);The IL-1β concentration in serum of SCOPD patients with high-risk group and SCOPD with low-risk group were 43.32±2.08pg/ml and 37.48±2.31pg/ml,Comparing SCOPD with low-risk group,the IL-1β concentration in serum of SCOPD patients with low-risk group were higher,the difference was significant(P<0.05).(4)TREM-1mRNA expression in peripheral blood mononuclear cell nucleus of COPD each group increased with the raising of sTREM-1 and IL-1β concentration in COPD patient serum,were positively correlated and the correlation were statistically significant respectively(r=0.78,P<0.01;r=0.82,P<0.01).The sTREM-1 and IL-1β concentration in each group of COPD Patients serum were positively correlated,the correlation coefficient r=0.81,the difference is statistically significant(P<0.01).(5)The expression of TREM-1mRNA in peripheral blood mononuclear cell nucleus and sTREM-1 and IL-1β concentration in serum of SCOPD patients,were raising with the lung function decline.The Correlations were negatively correlated and the correlation were statistically significant respectively(r=-0.67,P<0.01;r=-0.62,P<0.01;r=-0.69,P<0.01).Conclusions:TREM-1 and IL-1β may were involved in the COPD airway inflammation,and were positively correlated with the severity of the condition to the patients with COPD.(1)TREM-1mRNA expression in peripheral blood mononuclear cell nucleus of COPD and sTREM-1,IL-1β concentration in serum of COPD patients were higher than those of healthy control groups,suggesting that TREM-1,sTREM-1 and IL-1β were involved in the occurrence and development of COPD.(2)Along with the aggravation of COPD and the decline of lung function,TREM-1 mRNA expression in PBMCs and the concentration of sTREM-1 and IL-1β increased,suggesting that TREM-1,sTREM-1 and IL-1β were indicators of COPD severity assessment.(3)In each group of COPD,TREM-1 mRNA expression and the concentration of sTREM-1,IL-1β were positively correlated,they may in the same signaling pathways promote inflammation.
