The Clinical Value Of Central Retinal Artery And Internal Carotid Artery Ultrasonography In Early Diagnosis Of Diabetic Retinopathy

Objective: The hemodynamics of central retinal artery and internal carotid artery in diabetes mellitus(DM)patients were analyzed by color Doppler imaging to discuss the clinical value of ultrasound in diagnosis of Diabetic retinopathy(DR).Methods: 62 patients(124 eyes)with DM were enrolled in this study and divided into non-diabetic retinopathy(NDR)group and diabetic retinopathy(DR)group according to the results of fundus photograph,including 27patients(54 eyes)with NDR and 35 patients(70 eyes)with DR.Meanwhile,37 healthy volunteers(74 eyes)were selected as control group.The central retinal artery and internal carotid artery of DM patients and healthy volunteers were examined by ultrasound,the peak systolic velocity(PSV),end-diastolic velocity(EDV)and resistance index(RI)were assessed and compared.Results: The PSV of central retinal artery in three groups were:7.93±1.60cm/s、10.65±2.07cm/s、11.53±2.07cm/s,EDV:2.71±0.57 cm/s、4.14±0.83cm/s、4.72±1.07cm/s,RI:0.66±0.06、0.61±0.03、0.59±0.05;The PSV of internal carotid artery in three groups were: 52.64±9.00cm/s 、57.62±6.11cm/s 、 56.38±11.19cm/s,EDV : 20.34±5.17cm/s 、 24.83±5.14cm/s、24.42±5.64cm/s,RI:0.62±0.08、0.57±0.07、0.57±0.06.As compared to controls,the PSV and EDV of central retinal artery in patients with NDR were lower,meanwhile,the RI was higher(PSVP<0.05,EDV and RI P<0.01).Patients with DR had significantly lower PSV,EDV and higher RI comparing to patients with NDR and controls(P < 0.01).No statistical difference was found among patients with NDR and controls regards to PSV,EDV and RI of internal carotid artery(P > 0.05).However,there was a significant reduction in PSV,EDV and increase in RI of internal carotid artery between DR group and NDR group along with controls(compared to NDR group,PSV、EDV and RI P<0.01;compared to controls,PSVP<0.05,EDV and RI P<0.01).Conclusion: The hemodynamic parameters(PSV,EDV and RI)of central retinal artery and internal carotid artery measured by ultrasound may provide more diagnostic information for early clinical diagnosis and treatment of DR.
