Objective The study is aimed to investigate the association of macrophage polarization in tissues infected by Talaromyces marneffei with histopathology or clinical manifestation.Meanwhile,the preliminary mechanism of Talaromyces marneffei escaping from host immune defense is aimed to be clarified.Methods Formalin fixed paraffin-embeded tissues of skin lesions from16 patients with HIV(-)infected by Talaromyces marneffei were collected.At the same time,formallin fixed paraffin-embeded tissues of mice infected by Talaromyces marneffei were also collected,including tissue from skin,lung,liver and spleen.The patterns of inflammatory reaction were assessed for the infected tissues by HE staining.Expressions of CD86,CD163 and Arg-1 of macrophages in infected tissues were detected by immunohistochemistry methods.Furthermore,clinical data of 16 patients infected by Talaromyces marneffei were retrospectively collected and the association with macrophages polarization was analyzed.Results 1.Histopathological examination for 16 cases showed 10 cases presented as suppurative inflammation(62.5%),5 cases presented as suppurative inflammation combined with granulomatous inflammation(31.35%)and 1 case presented as necrotic inflammation(6.25%).The infiltrating inflammatory cells were a mixture of neutrophils,eosinophils,plasmocytes,lymphocytes and macrophages.Histopathological examination of infected tissues from 10 mice all showed suppurative and granulomatous inflammation.The infiltrating inflammatory cells were a mixture of neutrophils,plasmocytes,lymphocytes and macrophages,but without eosinophils.2.Expressions of CD163 in macrophages were observed in skin tissues from 16 cases infected by Talaromyces marneffei.Expressions of CD86 in macrophages were observed in7 cases(43.75%),and only 1 case(6.25%)with expression of Arg-1.However,expressions of CD86,CD163 and Arg-1 in macrophages were all observed in infected tissues from 10 mice(including liver,lung and spleen).3.The median(quartile)ratios of CD86/CD163 intensities was 0.00(0.00,0.51)in the patient group and the average ratios of CD86/CD163 intensities was 1.71±1.91 in the mice group.There was significant difference between the two groups(P<0.05).The average count of lymphocytes was 3.01 ± 0.94 in patients with CD86 positive,but that was 1.87±1.05 in patients with CD86 negative.There was significant difference between CD86 positive group and CD86 negative group(P<0.05).Conclusion There may have an imbalance of macrophages polarization in the skin lesion of HIV(-)patients infected by Talaromyces marneffei,and macrophages have a tendency to be manipulated to M2 phenotype.
