Disease Burden Of Acute Gastroenteritis During Autumn And Winter Among Children Under 5 Years Old In Henan Suixian

Objective:1.To understand the current status,etiology,influencing factors and clinical behaviors of acute gastroenteritis of children in autumn and winter.2.Assess the burden of acute gastroenteritis disease in children in autumn and winter,calculate the local DALYs and financial burden.3.To analyze the prevalence of epidemic status,morbidity,doctor visits and disease burden in different age groups and different symptom types.Method:1.Through the literature,statistical yearbook,government reports and official statistics,collecting the total number of local children under the age of five,each age distribution of them,and the basic information such as the local GDP per capita.By asking the doctor,fill out the medical organization’s questionnaire,and learn the basic information and treatment of diarrhea cases in local medical institutions and related clinics.All children under the age of five in 20 randomly selected communities were included in the study.2.The investigation was carried out in the county of suxian in Henan province in December 2016,and the survey was carried out in a unified survey.Of families and individuals(including basic data,the incidence data of last 3 months of acute gastroenteritis(symptoms,duration,etc.),medical conditions(JiuZhenLv,hospitalization rate,doctor hospital,hospitalization days,etc.),behavior,coping styles,treatment,medical expenses,medical expenses and delays days,acute gastroenteritis effect factors(other diseases,health habits,cultural factors,information way,etc.).3.Use EpiData3.1 to set up the database,data review,and logical check.For data collection and analysis,using Excel2013 and SPSS20.0,acute gastroenteritis,basic features of the survey population characteristics,medical behavior and characteristics,the economic burden of descriptive analysis,the related influence factors are chi-square test,analysis of variance and rank and inspection,inspection level of 0.05.The disease burden of acute gastroenteritis was reflected in the two weeks of annual rate,the year of disability adjusted life(DALY),direct economic burden and indirect economic burden.Results:1.The survey included 1,582 valid respondents,with the disease burden data from 260 subjects,and the most recent onset of gastroenteritis in 3 months.In two weeks,the acute gastritis had 102 new cases,with a rate of 6.4 percent for two weeks.To reflect the genuine Sui County the onset of acute gastroenteritis in the autumn and winter of children under the age of five levels,according to the actual number Sui County all age groups and the proportion,for correction,for two weeks to develop rate suffering from acute gastroenteritis two weeks after correction rate was 10.4%.The incidence of different age groups with age is on the decline,forincidence of two weeks,2 years old and above is similar to boys and girls,boys than girls under the age of 2.2.The survey for nearly three months of 260 cases of acute gastroenteritis cases,221 cases with diarrhea(85%)and 103 cases with vomiting(39.6%),64 cases had symptoms of fever(24.6%);Of these cases,only 157 cases of diarrhoea(60.4%),only 39 cases of vomiting(15.0%),and 64 cases(24.6%)of diarrhoea and vomiting.Febrile symptoms were not uniform in cases of only diarrhoea,vomiting,diarrhoea and vomiting,and different types of cases were distributed in different age groups.3.The median duration of acute gastroenteritis in children under 5 years of age is 2.92 days,and children between 0 and 1 years of age have an average duration of 3.94 days,and the average duration of a child’s illness between the ages of 2 and 4 is 2.19 days.4.Cases of acute gastroenteritis have an average of 3.19 days of diarrhoeal disease.Vomiting lasts for an average of 1.70 days,and diarrhea usually lasts longer than vomiting.Children from 0 to 1 years of age were on average 1.5 days longer than those between 2 and 4 years of age,and the maximum number of episodes of diarrhoea and vomiting was more than 2 to 4 years,and the indirect response was more severe.5.Of the 260 cases of acute gastroenteritis in the survey,194 patients went to the clinic and 74.6%in the diagnosis rate.Among them,six were hospitalized,the hospitalization rate was 2.3 percent,the diagnosis rate was no gender difference,and the low age group was more than the high age group.Due to the grassroots health institutions to see the doctor is relatively convenient,cheap,issued in 145 patients(74.8%)in the first choice of village clinics or individual clinic,the investigation of cases of acute gastroenteritis in the process of visits,the treatment of medical institutions to take measures to take medicine to give priority to(70.1%).6.There were no deaths in the survey,and the loss of life years(YLLs)was 0,so the survey’s DALYs = YLDs.The burden of acute gastroenteric disease of 72,507 children under 5 years old in Sui County,in the autumn and winter,is 219.92 DALYs.In Sui County of Henan,DALYs showed a tendency to decline with age.DALYs in different age groups were significantly different.Among them,the 0-year-olds were the highest,with the DALY loss of 43.9%of children under five years old.The 1-year group had a loss of 36.4%after the age of 0.The loss of DALY,ages 2,3 and 4,was relatively low,with 3 age groups totaling 43.40 DALYs(19.7%).7.Children under the age of 5 years old of acute gastroenteritis in the autumn and winter of financial burden of the sample per capita is 261.72 yuan,the economic burden negatively correlated with age,the economic burden of hospitalized cases were significantly higher than that of the rest of the population,is outpatient cases more than 10 times,and is no clinical visit cases more than 50 times.For per capita economic burden of different symptom types of cases,cases of diarrhea and vomiting was obviously higher than that of only vomiting cases or only diarrhea cases,but there is no difference between the economic burden of only vomiting cases and only diarrhea cases.8.After correction with Sui County actual population and incidence of children under the age of five,the economic burden is 304.71 yuan per capita,the adjusted economic burden of children under 2 years old(0～1 year old group)is 363.65 yuan per capita.Estimate Sui County children under the age of five a quarter(3 months),the onset of acute gastroenteritis 48540 person-time,acute gastroenteritis a quarter of children under 5 years old(3 months)to the economic burden of Henan Sui County totaled 14.7906 million yuan,accounting for Sui County quarter gross national product(GNP)of 4.35 per thousand.The economic burden is concentrated in the 0-1 year group,and the economic burden of acute gastroenteritis in children under 2 years old is 3.72 per thousand of the country’s gross domestic product.Conclusion:The incidence of acute gastroenteritis in children under 5 years old is higher,and the incidence of the disease decreases with age.DALYs also declines with age.The vast majority of DALYs are concentrated in children under 2 years old.The age is negatively correlated with the economic burden,and the economic burden mainly focuses on the 0-1 age group.
