Objective:External ear reconstruction is one of the most difficult operations in plastic surgery.Once the complications occured,we should timely and correctly handle these complications,to achieve better results.Necrosis of coverage skin of the framework is one of the postoperative complicaitons.Improper management may cause the cartilage exposed, and even seriously infection and the framework have to be taken out.We summarize one year and two months cases who occurs exposure of cartilage framework,to guide clinical reference. Methods:This retrospective study included 367 cases(374 ears)in our department treated from January 2016.to February 2017 who receive stage 2 surgery, male 260,female 107,lef’t 123,right 23,bilateral 7,aged from 5 to 37 years, average age 10.6 years, years, the follow-up period average 3 months. There are three cases suffer necrosis of coverage skin of the framework exposure and they need to be under treatment in hospital. Results:The total incidence rate was 0.82%.One of the three cases received local fascial flap transfer with free skin grafting, one of them received temporoparietal fascia flap and skin graft and the last one was directly sutured after debridement. To the follow-up period, all of the three wound had been healing well, and had never happened skin necrosis and the framework exposure again. Conclusion:After the debridement, direct suturing, temporoparietal fascia with skin grafts or local fascial flap transfer with free skin grafting are effective treatments for necrosis of coverage skin of the framework, owing to its’simple operation, better prognosis and lesser impact.
