Effect Research On The Treatment Of Acute Gouty Arthritis With Damp-heat Accumulation Syndrome Rats Model By The Tongfeng Prescription

Objective:Under the guidance of the theory of traditional Chinese medicine(TCM)and the method of modern medical experiment,we established the rat model of acute gouty arthritis with damp-heat accumulation syndrome.According to the clinical experience of Professor Fang Dingya,we developed the Tongfeng prescription to cure the model rats.By investigating the swelling degree of the ankle joints and the changes of serum inflammation indexes before and after the test,we try to discuss the effect of Tongfeng prescription on the rat model and analyze its therapeutic mechanism in order to provide the basis for the clinical application.Methods:60 SPF male Wistar rats were randomly divided into 6 groups:normal group,model group,low dose TCM group,middle dose TCM group,high dose TCM group,western medicine group,and each group had 10 rats.The dosage of the low,middle and high dose TCM group was 0.585g/kg/d,0.17g/kg/d,2.34g/kg/d respectively.The western medicine group was treated with diclofenac sodium sustained release tablets under the dosage of 13.5mg/kg/d.Normal group and model group were treated with equal amount of normal saline(NS)for 18 days.Except for the normal group,the other groups were fed with high-glucose-fat food and honey water of the dosage of 200g/L,and oil(1g/100g)and 52%Red-star Erguotou(1ml/100g)alternatively.On the 11th day of the treatment,the rats were fed with 10ml/kg of Salmonella typhimurium and the half concentration of the bacteria 120 hours later except for the normal group rats,who were given the same amount of NS.On the 17th day of the treatment,the right ankle joints of the rats were injected with uric acid salt by the modified Coderre method except for the normal group rats’ right ankle joints,who were injected with the same amount of NS.After the 4h,8h,24h,48h of the modeling,the swelling degree of the ankle joints were measured by using the perimeter method and the toe volumes method,and the content of IL-1 p、TNF-α TGF-β1、CRP in serum were measured by using ELISA.The pathological changes of synovium were observed under microscope,which were cut from the right ankle of rats.After the treatment,we detect the hepatic-renal function of all rats to object the safety of drugs.Results:1.the rats’ ankle joints injected with monosodium urate crystals appeared swelling,heat pain,different degree of joint flexion,extension,walking slower,even tripod gait at the peak of inflammation,revealing the success of acute gouty arthritis modeling.The rats fed with Salmonella typhimurium appeared loose and rough hair,scrotal sagging,burnout,addicted to lying,slow action,loose stools,perianal contamination and high rectal temperature,revealing the success of damp-heat accumulation syndrome modeling..The swelling degree reached peak at 24h after modeling and alleviated inordinately then.The swelling degree of model group was highest,while middle dose TCM was lowest at 24h after modeling.There were significant differences between model group and normal group(P<0.05)on IL-1β,TNF-α,TGF-β1,UA and CRP,which indicated the success of the modeling.There was no statistical difference between western group and middle dose TCM group(P>0.05)on TGF-β1,which demonstrated that the effect of middle dose TCM group was similar to that of western medicine group.So as to high dose TCM group.There was no significant difference between TCM group and western medicine group(P>0.05)on IL-1β、TNF-α、UA、CRP,which showed that the effect of TCM group on the above indexes was comparable with that of the western medicine group.TCM group were superior to western medicine group on hepatic-renal function indexes.Overall,the curative effect of TCM group on the treatment of acute gouty arthritis was not weaker than that of western medicine group.Histological changes of synovial tissue pathology:compared with the normal group,the model group’s synovial cells appeared proliferate and congestive and lymphocytic infiltrated.There was a little lymphocyte infiltrating and we can occasional found synovial cell hyperplasia and vascular congestion in the western medicine group.The middle dose TCM group and the high dose TCM group are equal to the western medicine group,while the low dose TCM group was slightly better than the model group.Conclusion:1.The rat model of acute gouty arthritis established by using the modified Coderre method can show the basic symptoms of acute gouty arthritis,and the rat model of damp-heat accumulation syndrome established by being fed with high-glucose-fat food,honey water and Salmonella typhimurium twice can show the basic characteristics of accumulation damp-heat syndrome.2.Tongfeng prescription can improve the joints swelling and synovial pathological changes in rats with acute gouty arthritis induced by MSU.The mechanism may be related to the inhibition of the expression of TNF-a and IL-1β,and the promotion of the expression of TGF-β1.
