This dissertation consists of three parts:literature review,clinical investigation and experience of my instructor treatment.1The literature reviewThe literature review consists the etiologies,pathogenesis and treatment progresses of vitiligo from the modernmedicine and traditional Chinese medicine respectively.And the conclusion is that vitiligo is a multi-factor disease which the exact cause is not clear,the clinical treatment is vary and comprehensive.2Clinical investigationBackground Vitiligo is a kind of disfigurement disease which can occur at any age and involving the exposed parts.It makes the patients lack self-confidence and do not want to social so that the patients have a great psychological burden and a poor quality of life.Because of vitiligo etiology is unknown so there is no special treatment.It is a big problem in the medical science.From the clinical observation it seems that patients with vitiligo is increasing recent years.Therefore,seeking effective treatment by the clinical rearch to explore its etiology and pathogens is patients’ and medical workers’common pressing needs.Objective Through the clinical data collecting by the vitiligo patients to analyzing the age distribution of vitiligo patients,the existence of gender differences,the popular season,whether the family had genetic status and clinical classification,staging and severity of skin lesions.To find the association of vitiligo with infection,drug-induced VitC,mechanical stimulation and other external factors,diet,daily habits and other factors.Methods The clinical data of vitiligo patients were collected from the clinical investigation of patients in Xiyuan Hospital,Chinese Academy of Traditional Chinese Medicine.It included gender,age,initial age,primary parts,the onset and progress season,family history,present patches and the sizes,type,staging,isomorphic reaction,white spot hair,causes,concomitant diseases,clinical treatments and so on.After the data collection it will be recorded in the Excel form.And then the data will be analyzed with SPSS23.0.Finally the conclusion will be drawn.Results 101 patients with vitiligo were included in this study,including 47 males,54 females,male:female 1:1.1.The age of onset was ranging from 5 to 59 years old.The average age of onset was 27.23 ± 14.14 years.The average age of males were 26.29 ±14.38 years old and females were 28.06 ± 14.35 years old.There was no significant difference in age between males and females.The initial onset age was at the peak of 21 to 30 years old.Among the 94 patients who could recall the initial onset and the obvious progression season,the highest proportion in summer was 56.4%,followed by spring,accounting for 26.7%,and 6.9%and 3.0%in autumn and winter respectively.Patients with incipient parts of the head,neck,hands,feet and other exposed parts accounted for 66.3%,other parts accounted for 33.7%.12.9%(13 cases)had positive family history among the 101 patients.The closer the blood relationship was,the higher the incidence was.And the first-degree relatives accounted for 50.0%(7 people),the second-level relatives accounted for 35.7%(5 people),the third-level was 14.3%(2 people).Vitiligo patients with non-segmental type(87 cases,86.1%)disseminated the most,accounting for 66.3%,followed by the face-limb type,accounting for 17.8%.86.1%(87 cases)patients were in progress and only 13.9%(14 cases)were stable.The lesion area of patients was mild-to-moderate accounting for 95%.26.7%of the patients had isomorphous reactions and 33.7%of the patients had the phenomenon of white hair.A total of 29 patients(28.7%)were associated with other diseases.Among the various pathogenic factors,the highest proportion of patients was mental factors,up to 88.1%,taking over fruits accounting for 26.7%,dietary bias accounted for 23.8%,external factors and bad habits accounted for 15.8%,both of infection history and VitC accounted for 12.9%.In the mental factors,tension anxiety were the most common,54 cases,13 cases of mental trauma,the pressure of learning and life in 20 cases,34 cases of fatigue.Conclusion(1)There is no significant gender differences in the incidence of vitiligo.21 to 30-year-old people was high-risk group.(2)High incidence was in summer.Head,face,neck,hands,feet and other exposed parts were the most vulnerable.(3)The type of vitiligo in non-segmental was the main type,followed by the face-limbs.And most patients were in the advanced stage,main lesions were mild and moderate,about 1/3 of the patients had the phenomenon of white hair.(4)The incidence of vitiligo has intimate relationship with genetic and immune factors,the closer the blood relationship,the higher the incidence.(5)Mental factors is a very important factor in the pathogenesis of vitiligo.Bad habits,trauma,infection,VitC and other factors have an impact on the occurrence and development of the disease.3 Instructor treatment experienceYao Chunhai teacher treats vitiligo by nourishing the liver and kidney,dispelling wind and removing obstruction in the meridians and,regulating qi and blood.He changed medicine with different symptoms.Mr Yao usually combines traditional Chinese medicine with modern research and pays attention to daily recuperate and always get satisfied effect.
