Versajet hydrosurgery system,water-jet,first appeared in the industry,occurred in 1960,mainly used in industrial cutting.Because of its non-thermal effect,does not change the physical properties of the material,no particulate pollution,smooth cutting edge and other advantages,it is widely used.In 1982,the two German professors puchriston and barters for the first time the water knife innovative application of surgery and found that due to liver tissue and intrahepatic blood vessels on the impact of different water load tolerance,can clear the liver tissue at the same time,More blood vessels,greatly reducing intraoperative blood loss,while significantly shortening the operation time.Subsequently,the water knife began to apply in various clinical areas,including liver surgery,neurosurgery,orthopedics,urology,orthopedics,maxillofacial surgery and ophthalmology.Cutting is an important step in surgery,but also a lot of plastic surgery and the key part of the key operations to determine the success or failure of surgery.The basic principle of waterjet is the application of high pressure principle of water molecules into a very high kinetic energy of the particles,through the narrow nozzle,the contact surface of the object to be washed to achieve the effect of cutting.Based on the body of the various organizations of different water content,organizational density,cell arrangement differences and other reasons,resulting in its high-speed flow erosion tolerance are significantly different.In view of the characteristics of tissue selectivity and protection of water knife height,this study intends to use waterjet to carry out the study of common diseases of plastic surgery.Part 1.The use of The Versajet hydrosurgery system to remove the injection of breast augmentation material polyacrylamide hydrogel(PAHG)clinical control studyObjetive:To clarify the effect and effect of Versajet hydrosurgery system on the removal of PAHG.Methods: A total of 64 patients with PAHG removal were enrolled from July 2014 to September 2016.The study has been approved by the Ethics Hospital of the Third Military Medical University.Including 30 cases in group A and 34 cases in group B Group A was the experimental group.The rats were treated with water knife combined with endoscopy to remove the injection of PAHG prosthesis,while group B was the control group,and the patients underwent routine swelling and aspiration.The postoperative observation indexes were: operation time,postoperative drainage volume,postoperative complications,postoperative gel residual rate,and patient satisfaction.Applying statistical analysis to objective analysis.To obtain its advantages,innovation and feasibility,to provide a reference for the operation.Results: The incidence of postoperative complications in group A and group B were 0%(0 cases)and 5.9(2 cases)%,the results were not statistically significant P = 0.117.In the observation of postoperative gel retention rate,observed in group A and B postoperative gel residue cases were 5 cases(16.7%)and 19 cases(55.9%),the two were statistically significant P = 0.001 The In addition,the satisfaction rate of patients in group A and group B was 28 cases(93.3%)and 22(64.7%)respectively.There was significant difference between the two groups(P = 0.006).There were no significant differences between the two groups during the time of surgery,hospitalization,and drainage.Conclusion: The water-jet combined with endoscopic clear injection of amniotic membrane material can remove the free injection material quickly and efficiently,reduce its residual rate and protect the surrounding normal tissue from damage.Is a worthy of the application of the surgical approach.Part 2.The hydrodynamic removal system in the treatment of axillary acupunctureObjetive:To observe the effect of using hydrodynamic scavenging system to remove topsoil to cure axillary osmolality.Methods: We retrospectively analyzed and analyzed a total of 41 cases of axillary rhizotomy from all eyes of the surgery from May 2014 to August 2016.The main indicators were: operation time,postoperative complications,postoperative efficiency,patient satisfaction,and postoperative recurrence rate.Results: The mean operative time for 41 patients was 98 ± 28(mean ± standard deviation).3 cases(7.3%)patients after local infection,after active anti-infection and after several dressing has improved.No local skin necrosis,bleeding after surgery,subcutaneous fluid and other complications.Three months after the evaluation of efficacy,which cured 36 cases(90.2%),markedly effective in 4 cases(9.8%),the effective rate of 100%.After six months to 1 year review,only 1 case of recurrence,the recurrence rate was only 0.2%.All patients had no surgical scar after surgery,and located at the top of the axilla,more subtle,to achieve a beautiful effect.Postoperative double upper limb activity.Conclusion: A total of 41 patients with axillary osmidrosis were treated with debride waterjet.All of them were effective and the clinical effect was satisfactory.Part 3.The hydrodynamic removal system in acute and chronic wound debridement of the clinical effect analysisObjetive: To observe the effect and function of hydrodynamic scavenging system in acute and chronic wounds.Methods: We retrospectively analyzed and analyzed 65 patients with acute and chronic wounds from April 2015 to July 2016.The main indicators were surgical incision,operation time,postoperative morbidity,patient satisfaction,and postoperative recurrence rate.Results: The mean wound debridement time was(32 ± 13)min in 65 patients,with an average debridement time of 1.5 ± 0.3 min per 1% TBSA.The positive rate of postoperative secretions was 10.8%(7 / 65),including Staphylococcus aureus in 3 cases,Klebsiella pneumoniae 2 cases,Pseudomonas aeruginosa in 1 case,mixed infection in 1 case,after active anti-infection treatment improved.The rate of skin grafting was 58.5%(38/65).All skin grafts survived and the survival rate was 100%.All wounds were healed and no resection was required.The average healing time was(13.5 ± 4.2)days.Conclusion: The water-jet is used for acute and chronic wound debridement.It can effectively remove the necrotic tissue and protect the normal tissue,which is beneficial to the survival of the skin and accelerate the wound healing.It provides a new choice for wound operation.
