Background The morbidity and mortality of malignant tumors are increasing year by year,and the majority of patients when treatment has been advanced,which seriously affect the quality of life and health of human beings.Now a number of studies have demonstrated that multiple metastases and thrombosis is the two major cause of death in patients with malignant tumors,and tumor development,invasion and metastasis of the body and abnormal blood coagulation are closely linked.In 2015,American Society of Clinical Oncology(ASCO)clinical guidelines suggest that most patients with malignant tumors during the hospitalization are are required to monitor changes in coagulation,regularly assess the risk of thrombosis,prophylactic anticoagulant therapy can prolong the survival of patients,improve the quality of life.Therefore,it is very important to identify and judge the abnormality of blood coagulation,and has become the focus of research and attention of domestic and foreign scholars.Objective The objective is to study the relationship between blood coagulation abnormalities and clinical features in patients with malignant tumor,in order to provide theoretical basis for avoiding thrombosis,assessing tumor occurrence and controlling the progression of the disease.Methods 166 patients who were diagnosed with malignant tumor by the pathology are divided into malignant tumor group and 66 cases healthy people as control group in the third affiliated hospital of Xinxiang Medical University from September 2014 to January 2017.The patients with malignant tumor were divided into these groups according to the clinical features,such as: according to the patient was treated first or recurrent would be divided into initial group and comprehensive treatment group,according to whether the occurrence of metastasis would be divided into metastasis group and no-metastasis group,according to tumor stage would be divided into stage I~II group and stage III~IV group,according to the existence of hypercoagulable state would be divided into hypercoagulable state group and no-hypercoagulable state group.And collect the clinical data and the general condition of the patient,including the indicator: the levels of PLT,PT,APTT,FIB,D-D and gender,age,metastasis(lung metastasis,bone metastasis,lymph node metastasis),tumor stage(I~II group and stage III~IV group),anti-tumor therapy(previous chemotherapy,radiotherapy,surgical treatment),tumor location(esophageal cancer,lung cancer,colorectal cancer,gastric cancer,breast cancer,cervical cancer,endometrial cancer,ovarian cancer,pancreatic cancer,malignant lymphoma)and tumor system(breast cancer,gastrointestinal cancer,female reproductive system and lymphatic tumor of blood system).Using SPSS19.0 statistical software for statistical analysis of all data,measurement data using((?)±s),count data using(n),meet the homogeneity of variance using t test,multiple independent samples were compared using one-way ANOVA;and compared using χ~2 test.P<0.05 was considered as significant.To investigate whether there is coagulation disorder in patients with malignant tumor,and the relationship between the coagulant function abnormality state and the clinical features of the tumor.Results 1 The abnormal rate of coagulation and the abnormal rate of coagulation index in the malignant tumor group were significantly different from those in the control group(P<0.05).The results showed that patients with malignant tumors are in the coagulant function abnormality state.2 The levels of PLT,FIB and D-D in the malignant tumor group were significantly higher than those in the control group(P<0.01).The results showed that the levels of PLT,FIB and D-D were higher in the patients with malignant tumors,and were susceptible to coagulation disorder.3 The levels of PLT,FIB and D-D in initial and comprehensive treatment group were significantly higher than those in the control group(P<0.05),the levels of FIB in comprehensive treatment group were significantly higher than in initial treatment group(P<0.05).The results showed that both the initial and comprehensive patients were susceptible to coagulation abnormalities,particularly in comprehensive patients.4 The levels of PLT,FIB and D-D in no-metastasis and metastasis group were significantly higher than those in the control group(P<0.05),the levels of FIB in metastasis group was significantly higher than in no-metastasist group(P<0.01).The results showed that both the no-metastasis and metastasis patients were susceptible to coagulation abnormalities,particularly in metastasis patients.5 The levels of FIB in stage I~II higher than the control group(P<0.05),the levels of PLT,FIB and D-D in stage III~IV group was significantly higher than in the control group(P<0.01),the levels of FIB,D-D in Stage III~IV group was significantly higher than in Stage I~II group(P<0.01),The results showed that both the stage I~II and stage III~IV patients were susceptible to coagulation abnormalities,particularly in stage III~IV patients.6 The levels of FIB,D-D in five of tumor were higher than the control group(P<0.05),The level of PLT in lung cancer group,gastric cancer group and breast cancer group PLT was higher than the control group(P<0.05).The level of FIB in the esophageal cancer group,lung cancer group,colorectal cancer group and breast cancer group was higher than the control group(P<0.05).The results showed that the esophageal cancer,lung cancer,colorectal cancer,gastric cancer and breast cancer patients with varying degrees of coagulation abnormalities,the levels of FIB,D-D were significantly higher than other,and particularly gastric cancer patients are more prone to coagulant function abnormality state.7 The hypercoagulable state was common in patients with malignancy tumor,which had correlation with metastasis,staging,chemotherapy,tumor location and tumor system(P<0.05),but not with gender,age,lung metastasis,bone metastasis,lymph node metastasis,surgery and radiotherapy history(P>0.05).The results showed that hypercoagulable state of malignant tumors had correlation with metastasis,staging,chemotherapy,tumor location and tumor system.Conclusions 1 There are different degrees of coagulation function abnormality state in patients with malignant tumors,the levels of PLT,FIB and D-D were significantly higher.2 Hypercoagulable state of malignant tumors had correlation with metastasis,stage,anti-tumor therapy,tumor location and tumor system.
