Objective : To investigate the current prevalence situation of high-risk human papillomavirus infection and the risk factors of HR-HPV in Cangyuan County of Yunnan province. To provide scientific basis and guidance for the prevention of cervical cancer.Methods: Cangyuan Va Autonomous County of Yunnan province is a frontier minority autonomous county which mainly in The Va nationality and together with Dai/Han/Lahu/Yi nationality etc. We conducted the study using random sample of 488 women in the four towns of Cangyuan County and complete the questionnaire and screening of cervical cancer. Questionnaire included basic information, pregnancy history ,smoking, drinking, sexual behavior . Specimens were collected by DH3 HPV nucleic acid detection (hybrid capture - chemiluminescence method) and TCT technology, HPV-DNA > 1.0pg/ml was defined as HR-HPV positive. SPSS 21.0 software was used to statistical analysis. Chi-square test and unconditional Logistic regression model were used to evaluate the effects of different risk factors. Statistical significance was determined by P<0.05.Results: The overall infection rate of HR-HPV among 488 women was 6.56%.The prevalence of HR-HPV in different nationalities were: Va nationality 6.59%, Han nationality 8.05%, Dai nationality 5.56%, other nationalities 8.05% ,with no statistical significance for the infection rate in different nationalities (P > 0.05).The prevalence of HR-HPV in different diagnose of TCT were NILM 5.21%,ASCUS 80%、LSIL 100%、HSIL 100%, the positive rate of HR-HPV in different diagnosis have statistically significant (χ2 =88.19 , P<0.01) . There were no significant differences of HR-HPV infection in different age groups (P>0.05), age of 40~49 years have the lowest infection rate of 5.59%, over 65 years have the highest infection rate of 9.09%, younger than 29 years (7.27%) and older than 50 years (8.33%) is the two peak of HR-HPV infection. Chi-square test result suggests that HR- HPV infection correlate with age at first sexual intercourse (χ2=4.36, 0.01<P<0.05) and smoking(χ2=6.49, 0.01<P<0.05) . There were no statistical significant for the infection rate in pregnancy history, number of lifetime partners ,sexual frequency,drinking and education years. Multivariate Logistic regression analysis showed that: the risk factors of HR-HPV infection were first sexual intercourse (OR=2.257, 95%CI: 1.041,4.892)and smoking (OR=2.826, 95%CI:1.240,6.444).Conclusion: There was no significant correlation between HPV infection rates and nationality, age,pregnancy history, the number of lifetime partners,sexual frequency, drinking history and education years. Risk factors for HR-HPV infection were age at first sexual intercourse <20 years and smoking. There was two peaks in HR-HPVprevalence: younger than 29 years and older than 50 years. The combination of TCT and HPV screening, can effective shunt high-risk groups of cervical cancer,improve the correct rate of diagnosing cervical lesions.In prevention and treatment of cervical cancer, especially in rural of ethnic minorities, measures should be taken targeted at the high risk factors and the folk customs characteristic of ethnic,strengthen the education of cervical cancer prevention ,attach importance to the cervical cancer screening among susceptible population will have great significance to the prevention and treatment of cervical cancer.
